I am willingly giving my consent for sending my son to try-out and play basketball on July 26-29,
2022 (1pm-5pm) at Pinaripad Norte Gym. My son is in good health. I am fully aware of Covid-19
Pandemic and also fully aware of the precautionary measures to be taken while sending my son to
basketball try-out. I know that the school management and coaches will shepherd our son to safety but
we are agreeing that the school management, coaches and trainers will not be liable to any untoward
incidents beyond their control.
