VI. OBJECTIVES: The aims of this seminar- training are as follows:

 to prepare and update the teachers on the needs and
schedule of School and Division Checking of Forms;
 to provide a reliable assurance mechanism of learner
information;
 to ensure the quality and timeless of school reports; and
 to reduce the resources spent for clerical and records
management.

X. DESCRIPTION/
RATIONALE: Academic performance records, such as grades and report
cards are essential to the learners, parents and school. It provides
insight about student’s progress and helps parents and teachers
make important decisions regarding students' learning needs.
Attendance records also aide teachers and school staff maintain
accountability for students' safety.

Among the critical activities conducted at the end of every

school year are the preparation and checking of school forms to
ensure the quality and consistency of learner information. This
secures the consistency and reliability of the system in generating
forms which contain information that is accurate, updated, and
useful for evidence-based assessment, planning, and allocation of

These school forms and reports include most of the data

related to the school and students Thus, the Department of
Education (DepEd) officially released these DepEd School Forms
(SF) Templates to help the teachers easily consolidate the data for
the whole school year.

Prior to the implementation of DepEd Order No. 11, S. 2018 –

GUIDELINES ON THE PREPARATION AND CHECKING OF SCHOOL
FORMS, GEIS will conduct a School-Based Seminar-Training to
prepare and update the teachers on the needs and schedule of
School and Division Checking of Forms, provide a reliable
assurance mechanism of learner information, ensure the quality
and timeless of school reports, and reduce the resources spent for
clerical and records management.
