Reasons for Request:

I have the honor to request transfer from my present station, Sta. Maria National High School of
Schools Division of Bulacan to Schools Division of Baguio City for the following reasons stated herein
based on Deped Order 22 s. 2013 particularly RA 4670 and PD No. 603, Child and Youth Welfare Code:

1. After being married, I will be moving to Baguio City where my husband resides and for this
reason, I wish to be assigned to a school near to our residence. If I will be transferred, I could
save a significant amount enough to augment our income in defraying family expenses.
2. Sta. Maria National High School, is very far away from Baguio City and it will be impossible for
me to be at our home daily. As a nursing mother, it would be very hard for me to attend to the
needs of my baby. It is my desire to take good care of my child.
3. A considerable amount of time will be spent in commuting from my place of residence to my
present work assignment. This is economically and physically exhausting on my part. The time to
be saved from commuting can be used in preparing teaching materials for my lessons, hence,
resulting in enhanced quality teaching as well as spend more time with my family.

I am looking forward to your timely favorable consideration for this request which, when granted,
will be beneficial to the school, to the students and to the undersigned as teacher/public servant.

I am forever thankful for the learnings, experiences and challenges given by Sta. Maria National
High School. Thank you very much!
