AFFIDAVIT OF GUARDIANSHIP AND

CONSENT TO GET MARRIED

I, OLIVIA MARTINEZ PILOTA, of legal age, Filipino and resident of Block 17 Lot 17
Green Meadows Punturin, Valenzuela City, after having duly sworn to in accordance with
law do hereby depose and state:

1. That I am the Auntie and actual custodian of JHON DANTE A. MARTINEZ and he is
planning to get married;

2. That his father, DANTE O. MARTINEZ is my brother and he is presently working at

Masbate, and I am the one who is taking good care of my nephew while his father is
working in their province at Masbate;

3. That because of the foregoing his father cannot sign necessary papers/documents
needed for the upcoming wedding of my said nephew and I hereby declare to be his
guardian to be able to assist and accompany him and I am the one who will sign all
the documents/papers for his wedding;

4. That furthermore, I am giving my full support and consent to my said nephew to get
married;

5. That I am executing this affidavit to confirm and affirm the foregoing facts and to
prove that I am the legal guardian of the above-named JHON DANTE A. MARTINEZ
and for whatever legal purposes it may serve.
