AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, RAMONA S. HERNANDEZ and JEAN A. JAMERO, both of legal age, Filipinos, and
residents of 151 Cabilogan St., Bugo, Cagayan de Oro City and Johndorf Subd., Barra,
Opol, Misamis Oriental, after having been sworn to in accordance with law hereby depose
and say;

1. That we personally know MARIA DONNA G. SABALO, who is the daughter of

Jesusa V. Ganza and Antonino G. Ganza;

2. That her name as appearing in her Marriage Certificate is DONNA V. GANZA

whereas in her records in SSS, her name appeared as MARIA DONNA G. SABALO;

3. That the reason why her first name is spelled as DONNA instead of MARIA
DONNA in her Marriage Certificate was due to oversight of the person who
prepared the document in ascertaining the real spelling of her first name;

4. That in all her documents, she used the name MARIA DONNA G. SABALO, as it is
the correct spelling of her name;

5. That DONNA V. GANZA in her Marriage Certificate and MARIA DONNA G. SABALO
in her records in SSS refer to one, single and the same person and there is no
other person in our locality with the same name as that of her.;

6. That we know the foregoing facts because we are long time neighbors and friends
of MARIA DONNA G. SABALO;

7. That we are not related to her either by consanguinity or affinity and has no
interest in whatever this affidavit could be of use either directly or indirectly;

8. That we execute this affidavit to attest the truthfulness of the foregoing facts and
establish that DONNA V. GANZA and MARIA DONNA G. SABALO refer to one,
single and the same person, and for whatever legal purpose this affidavit may
serve.
