EASYJET VACCINATION DECLARATION

As part of your role with easyJet, it is recommended that you have had the measles
vaccination prior to employment commencing. While this is not mandatory, if you are not
immune, we recommend you contact your GP/healthcare practitioner to discuss the risks and
benefits of immunisation before making your own decision whether to be immunised.
Having discussed with your medical practitioner, it is your decision whether to proceed with
vaccination and, if so, your responsibility to identify a suitable pharmacy and/or practitioner
to provide and administer the vaccination to you in the months before you join us.
Please sign the declaration below to confirm that either:
(i) you have previously received the measles vaccine; or (ii) you have not received the
vaccine and do not wish to, in which case by signing the below declaration you confirm that
you fully understand and accept the risks of not being vaccinated and performing your role
with us; or
(iii) you have not received the vaccination but are making arrangements to do so in which
case you confirm that you will make arrangements to receive the vaccination by your start
date and you fully understand and accept the risks of performing your role with us before
your vaccinations are complete.
