1. INTRODUCTION
Having gone through the Blocks 1 and 2 of this Practical course (BNSL-040), you must have
understood as to what activities you will have to practice and perform. Some of the activities are to
be performed at your Programme study centre called Supervised Activities during the practical
contact sessions. Some of the activities have to be performed on your own at your home, study
groups, with friends and in your neighbourhood are called as Self Activities.

For Supervised activities, you will have to attend counseling and contact sessions at your programme
study centre, where programme incharge and academic counselors will demonstrate you the various
skills that you need to develop. The academic counselors will also guide you to practice these
skills/activities. For Self activities you will have to practice them on your own. These activities
have been clearly marked in your Practical Blocks and also provided in List of self and supervised
activities in this Log- Book.

You have to make record of the day-to-day (self and supervised) activities in this Log-Book and
submit this for evaluation at least one month before the final practical examination ( probably in
month of April for January session students and October for July session students) . For your
knowledge final practical examination will be conducted one month ( in month of May or November)
before final term end theory examination ( held in June or December). The self and supervised
activities will carry 50% weightage (25% self-activities and 25% supervised activities). Before
doing self and supervised activities you must refer the practical blocks and then start doing and
recording the activities. In some practical activities you have to stick photographs of your work for
which you can simply click photo from mobile and take a print out and stick it in the space given.
We hope that it will be very easy for you to do this. Otherwise hard copy of photo taken from
camera can also be pasted. The proforma and guidelines which you will use for doing practical
activities are enclosed at the end of each activity and you will have to record your action into these
forms/proforma wherever required.

Academic Counsellors will clear all your doubts during practical activities or otherwise whenever
required.

We hope you will get good practical learning experience while working through this Log-Book.

2. GENERAL INSTRUCTIONS TO THE STUDENTS
This Log-Book is a compulsory component of practical course. You are required to maintain record
of all the learning activities that you perform as a part of this course in this Log-Book. The Log-
book contains self and supervised activities. We have provided the guidelines and proformas for all
the activities. You need to record all the activities in the proforma.

2.1 Objectives of the Log-book


The objectives of the log-book are to:

• Provide you the guidelines for performing the activities;

• Enable the counselors to have first hand information about the activities performed by you;

• Help you in planning your activities in advance so that you can complete them within the time
frame; and

• Document your practical experience towards the completion of practical component of this
programme.

2.2 How to Use the Log-book


• Read practical manuals of the practical course thoroughly i.e. Block 1 and 2 of this practical
course.

• Refer the list of self and supervised activities given in the initial pages of your Log-Book.

• Refer the guidelines for undergoing the self and supervised activities.

• Read each activity and refer the relevant practical for detailed guidance.

• Record the activities in the Proformas.

• For recording self-activities you have to perform these activities yourself and fill up the proforma
provided in the log-book.

Please ensure that whenever you are performing any supervised activity at programme
study centre, you must get it countersigned by the counselor under whom you have carried
out the activities then and there.

Scenario: Mr. Ramesh has been hit by a car from behind at the City centre Practical Block 1
today at 10 am at a spot where there is running traffic. His age is 50 years Course
and he lives at northern point. He has reported his phone number to be BNSL-040
9999912455 and consents for the first aid. Upon collecting history you find
that he is allergic to sea food, is conscious , has fast pulse of 100/min, rapid respiration of 30/
min and warm body. Head to toe examination reveals bleeding from open wound with
tenderness. You call for an ambulance, control bleeding and use dressing on the bleeding
area.

Fill up the first aid treatment chart for the victim. Guidelines have been provided here.

Guidelines
• Refer Activity 2 of Unit 1 of Practical Block 1.
• Observe and Practice recording of First Aid Treatment Chart/Form.
• Understand the items to be filled in the First Aid treatment Chart/Form as follows:

1 Name of treating person : write your name as you are the first aid provider.

2 Location, date, time as per the area of emergency.

3 Surname, Name, Gender , address , phone number as provided by the victim.

4 Write history of illness as stated by the victim , what can you do and you have done for scene
safety , first aid assessment , past medical history of the victim.

5 Write down general observations, his response, pulse, respiration , temperature, head to toe
examination and injuries present.

6 Write the first aid provided.

7 Write the details of action e.g. whether the victim was taken to hospital by you or was an
ambulance called or if he was taken to his own doctor or any other service was called for at
that moment. Also fill up other related details like time you transported and to where.

8 If CPR or other first aid was continued and as per whose advice.This must also must be
mentioned.

9 Thereafter the first aid provider should fill up his/her details.

• Now, record your findings in the First Aid treatment form given here as per the above given
scenario.
