JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

RE: ONE AND THE SAME PERSON

We, LUCIA J. RABANSON and RUBY ANN B. OCHEA, both of legal age,
Filipinos, and residents of Isla Verde San Isedro Talisay City and Sta. Rosa Lapu-Lapu
City, respectively, after having been duly sworn to in accordance with law do hereby
depose and say:

1. That we personally know NILO BUTALID ABAIS, who was born on March
12, 1963. He is our long-time friend and neighbor;

2. That his true and correct full name is NILO ABUGADIE ABAIS as reflected
in his Baptismal Certificate and PhilHealth Records;

3. However, in the VSMMC Hospital Records his middle name was erroneously
recorded therein as “NILO BUTALID ABAIS” instead of “NILO
ABUGADIE ABAIS”.

4. That such error may have been caused by mere inadvertence of the person
who prepared his Hospital Records;

5. That the names “NILO BUTALID ABAIS” appearing in his Hospital Records
and “NILO ABUGADIE ABAIS” in the Baptismal Certificate and PhilHealth
Records, REFERS TO ONE AND THE SAME PERSON;

6. That we execute this affidavit to attest to the truthfulness of the foregoing

statements and for all legal intents and purposes this may serve.
