SUBJECf New Design of PbilHealth Identification Card

I. RATIONALE

Section 7 of the Revised Implementing Rules and Regulations of the National

Health Insurance Act of 2013 mandated that the Corporation shall assign a permanent
and unique PhilHealth Identification Number (PIN) to every enrolling Filipino
membersincluding their dependents.

Previously, the Corporation has issued different looks of Identification Cards ro

each membership category. Since this might create confusion among members and to set
a standard image of one of the Corporation's brand or identity, this circular is hereby
ISSued.

II . SCOPE

All members of me National Health Insurance Program (NHIP) shall use the new
design of the PhiJHealth Identification Card. It shall se.rve as the members'proof of
identification and membership registration.

III. DETAJLS OF THE NEW DESIGN OF THE PHILHEALTH

IDENTIFICATION CARD

fV. REPEALING CLAUSE

PhilHealth Circular no. 11, s. 2010, PhilHealth Circular No. 26, s. 2010, PhilHealth
Circular no. 024, s. 2012, Ph.i.IJ-lealth Circular 035, s. 2012 and PhilHealth Circular 0005,
s. 2014 are hereby repealed and/ or amended accordingly.

V. EFFECTIVITY

This Circular shall take effect fifteen days after publication in any newspaper of general
circulation and shall be deposited thereafter with the National Adm.i.nistrative Register at
the University of the Philippines
