JOINT AFFIDAVIT OF TWO DISINTERESTED PERSON

That we, NIEVES APORONG and LETTY RAMOS, both of legal ages, Filipino
citizens and both resident of Brgy. Tocok, San Fabian, Pangasinan, respectively, after having
been sworn to in accordance with law do hereby depose and state that:

1. That we personally know the person BETTY MENESES-VEGA, who was born
February 28, 1963 at Tocok Sapdaan, San Fabian, Pangasinan daughter of Sps. Jose
Meneses and Crisfina Torio (both deceased);

2. That it appear in her Certificate of Live Birth (PSA) her name and birth date is BETI
MENESES and born March 30, 1963, was mistakenly registered by person who
registered her name;

3. That the true and correct name and date of birth is BETTY MENESES-VEGA and born
February 28, 1963 which appear in her Social Security System records;

4. That we know BETI MENESES and BETTY MENESES refers to one and the same
person and she was born February 28, 1963 and not March 30 1963;

5. That we know all these facts for the reason that we are immediate neighbors and close
friend of the family;

6. That we are executing this affidavit to attest to the foregoing facts and for whatever legal
purposes and intents this may serve.

IN TRUTH WHEREOF, we have hereunto set our hands and affixed our signature this
