IV. PROCEDURES
A. Review/Introductory Provide each pupil with a copy of a Pupil Profile Form. Guide them in filling it out.
Activity

After filling out the form, explain the purpose of the activity and the value of filling it out
appropriately.

B. Activity/ Motivation Show pictures of different kinds of forms, and let the pupils guess each picture.

D. Abstraction Discuss:
A form is a printed or typed document with blank spaces for insertion of required
information, such as full name (surname, first name, middle name), birthdate, and address.
Different kinds of forms are provided for different purposes—from asking basic personal
information to answering surveys or questionnaires. Also, establishments, departments,
companies, or groups create their own forms, depending upon what data they need. Printed
forms come in various sizes and paper color. Similarly, online forms appear on your desktop
screens in numerous layouts. Another term for form is slip.

It is important for every individual to learn about how to properly fill out forms since this is
an integral part of our everyday life. Whether you are a student, a job applicant, or a citizen
who needs to request for a copy of birth certificate from PSA, one should be able to properly
and correctly provide all the information asked for in the form.

Remember: Here are some helpful reminders for filling out forms.

1. Read the form carefully before you write anything.


2. Make sure that you keep the necessary information or complete personal details.
3. Find out if you are asked to use block letters or block capitals, as in:

4. Find out if there is a need to fill in everything.


5. If an item does not apply to you, leave it blank or write N/A (not applicable).
6. If there is an item on the form you do not understand, ask for help.
7. Review the entries or date you have entered before submitting the form.

On Terms and Terminologies: Here are other relevant information that we need to keep in
mind.

1. Last Name, Surname, and Family Name have similar meanings.


2. First Name and Given Name are the same, relating to the name your parents gave you.
3. There is a difference between Middle Name and Middle Initial. If a form asks for your
middle name, you should completely spell out the maiden name of your mother (e.g.,
Ocampo, Reyes, Javier). If it asks for your middle initial, you only write the beginning letter
of your mother’s maiden name (e.g., O., R., J.).
4. Sex and Gender mean the same thing.
5. When a form has blocks wherein you need to write your name and/or other information,
you only write one letter per block. Skip one block to space out words within your complete
name and/or other information.

6. In relation to your birthdate (mm/dd/yyyy):
a. mm means month; you must enter two-digit numbers (e.g., 10, 11, 12). In cases where
your birth month fall under a one-digit month, write 0 before it (e.g., 01, 02, 03).
b. dd means day; you must enter two-digit numbers (e.g., 05, 15, 25).
c. yyyy means year; you must enter the complete year when you were born (e.g., 1998,
2000, 2015). If it is written like this, yy, write the last two digits of your birth year only (e.g.,
98, 00, 15).
7. Oftentimes, forms require letters to be written in capital letters. Be mindful of this detail.
8. “Fill out” and “fill in” are both acceptable when referring to answering the blanks in a
form. It is incorrect to say, “fill up.”

F. Generalization Remember:
A form is a printed or typed document with blank spaces for insertion of required
information, such as full name (surname, first name, middle name), birthdate, and address.
“Fill out” and “fill in” are both acceptable when referring to answering the blanks in a form. It
is incorrect to say, “fill up.” It is important for every individual to learn about how to
properly fill out forms since this is an integral part of our everyday life.
