    Temperature & Rapid-Antigen Test (RAT) Record Form

    Dear Parents/Guardians,

    1. CAIS students are required to have their temperature and RAT result recorded before leaving home. If the child
    has a fever or has a positive result, he/she must not attend school. Please inform the school office of absence
    and reason.
    2. Students who do not bring this form to school fully completed each day will not be permitted in classes.
    Parents will be contacted to collect their child from school.
    3. For those recovered from COVID-19 and do not have symptoms, they are not required to be tested RAT
    within three months from the date of the recovery. Date of recovery: __________________________
