1. How can customer subscribe

a. Customer will have to subscribe and pay for the connection. (130, 250 & 350 – 12 months’ connection
only)

b. After Payment by end of the day customer will receive SMS for enrollment form link.

Customer will have to fill up the form with details requested. (Customer will have to fill only 1 response
& customer can either enroll himself or anyone else for the insurance)

c. After form Fill-up by next day customer will receive an SMS that his policy is activated.

2. What kind of Insurance are we providing?

This is Accidental insurance and customer can claim benefits on case of Disablement, Death or
hospitalization due to accident.

3. Can customer enroll anyone else in the insurance?

Yes, customer will have insurance for one person while filling up the form customer can either enroll
himself or anyone else from his family for the insurance.

Customer will have fill-up (Name, Contact Number, Citizenship No.) detail for the person willing to
insure.

In any case customer will be allowed to get insurance for a single person only.

4. Can you get insurance in any other 1 month, 3month or BTL Plans?

No, this offer is only limited for 12-Months ATL plan subscription.
