Instructions for Completion


When completing the audit it is important to use the N/A and N/S if the equipment is not fitted to the vessel.
The auditor shall be on the bridge as an OBSERVER during navigation periods. During these periods the auditor must not
carry out auditing interviews.
Any officer being audited must not be the OOW at the time of the audit. For example the Second Officer may be observed
during his watch period but can only be questioned when other OOW is the watch officer.
If at any time whilst present on the bridge, the auditor in his professional opinion believes intervention is required to prevent
a dangerous situation developing, it is expected that he immediately makes his concerns known to the OOW.
Any observations and non-conformities that have been closed during the audit should still be included but the fact that the
observation has been closed should be stated in the audit report.
On completion of the audit all findings must be discussed with all OOW and the audit summary signed by all parties, a
scanned copy of this must be forwarded together with the audit report. A copy of the audit report must be inserted in ERP
system.
The Master has to respond to all observations and non-conformities raised. In order to comply with this requirement it is
suggested that he hold a formal meeting with the Bridge team to discuss them. This meeting is to be held when operationally
convenient, but it must be within a week of the audit being completed. The comments from this meeting are to be entered
into ERP system as corrective actions to the observations rose during audit.
