AFFIDAVIT

I,THATIKANTI JANARDHAN,S/o Lingaiah,Age:42 years,

Occ: Pvt Driver,Caste:Kukmmari,R/o H.No: 13-26,Peddsampet H/o
Jallaram,Kamanpur Mandal, karimnagar District, Do hereby
solemnly affirm and state on onth as under:-

1. That I have appeared for the secondary school Certificate public

Examination held in the year,1987 with SSC Marks Fail Memo
through ZPHS Godavarikhani,Ramagundam,Karimnagar District.

2. That the Original S.S.C Fail Memo was issued to me by Bord of

Secondary Education has been lost by me I was Proceeding to my
village from Shopping complex,8 Incine Colony in auto and in spite of
my best efforts I am unble to trace it and it is lost beyond recovery, In
case, if it is traced in future I shall submit it to the Board of
Secondary Education for cancellation.

That I am need for a Duplicate copy of the said S,S.C. Fail

Certificate for which purpose I am hereby making this declaration as
required by the secretary,Board of Secondary Education.
