Title of the Activity: Organization of CI Team- Project CREATE

Objective: To organize a CIP Team that will help grade 5 learners to enhance their writing and
reading skills through the proposed interventions.

Narrative Report:

The Grade 5 teachers convened at 9:00 in the morning with the presence of the ever-
supportive School Head Ma’am Jovelyn S. Felix. The meeting commenced with an opening
prayer by Mr. Richard SM. Cruz, a grade 5 teacher. It is followed by the singing of national
anthem through video presentation. Attendees shared their thoughts about the plan of organizing
a CIP Team to further improve the skills of the learners in constructing sentence as reading
enhancement.

On the onset of the meeting, Ma’am Jovelyn S. Felix discussed the least competencies
mastered in grade 5. It has found out that most of the pupils found difficulties in constructing
sentence involving subject verb agreement. She also shared the priorities of the school based on
the needs of the learners. The results of the different assessments conducted by the school and
the gaps and adjustments was also presented. Sharing of programs and projects was facilitated by
the school principal. All the ideas collated were put into proposed intervention program which is
the Project-Create.
The meeting ended successfully and collaboratively.

The CI Facilitator and CI Team conducted an interview to the parents and pupils on
found difficulty as shown in the consolidated result of 3rd Quarter, 1st Summative test in English.
The CI Team aims to identify the reason behind why most of the pupils obtained a failed scores
in the said examinations. All the data gathered where collated and documented as a basis for the
proposed interventions.

Title of the Activity: Identifying Storm Clouds and Administer Pre-Test

Objective: To identify priority improvement areas and administer pre-test in Grade 5- Quezon
that obtained the most number of pupils who are failed among the 3 section of grade 5.

Narrative Report:

After the 1st summative test in English was consolidated. It has found out that most the
pupils of grade 5- Quezon were not able to construct simple sentence involving subject-verb
agreement. To justify the result of the said examinations the CI Team administered pre-test
examination started at 10:00 a.m. The focus of the test is constructing a sentence involving
subject verb-agreement. After careful evaluation, only 9 pupils (19.14%) got a passing score out
47 learners. The CI Team accomplished the storm clouds where the reasons behind of those
failed scores can be found.

The CI facilitator and CI Team used all the available gathered information during interview.
The team accomplished why-why diagram that shows the reason why the learners are not able to
construct sentence. By that means, we can use the data at hand to formulate an effective and
responsive interventions program that will enhance the skills of the learners.

The Grade 5 teachers convened at 9:00 in the morning with the presence of the ever-
supportive School Head Ma’am Jovelyn S. Felix to hold a meeting with a parent of Grade 5
Quezon. The meeting started with an opening prayer by Mrs. Aileen Esteban, parent
representative from grade 5. It is followed by the singing of national anthem through video
presentation. Mr. Isaac Balagtas, a grade 5 teacher facilitated the checking of attendance, while
Mrs. Elsie Alceso, CI Team Leader gave her opening remarks for today’s meeting.
On the onset of the meeting, Mrs. Elsie Alceso, discussed the result of the summative test
in grade 5 to the parents and also the launching of Project CREATE. Mr. Michael C. Pascual
encouraged all the parents to support this program by giving them their duties and
responsibilities to make this innovation a successful one. At the end of the meeting, Mr. Isaac S.
Balagtas, Jr. a grade 5 teacher, gave his insights and closing remarks.

Narrative Report:

On April 25, 2023 at 10:00 am, our school head and resource personnel, Dr. Jovelyn S.
Felix set a meeting with CI Team to finalize the improvement plan that need to prioritize among
the pupils of grade 5 Quezon. This is to ensure that all interventions to be given by the team are
appropriate in order to achieve desired outcomes.
In line with this, the team also identified the activities, target date, person involved, funds,
and expected outcome at the end of every session.
The CI Facilitator, Mrs. Elsie A. Alceso, emphasized the use of pictures, learning activity
sheets, portfolio, and progress chart as part of the remediation process. Mr. Michael C. Pascual
assured that all the parents will take part in this project as one of the major stakeholders. In the
end of this program, Mr. Isaac S. Balagtas, Jr review the minutes for today’s meeting.

On this day, the CI team convened a meeting at 10:00 am in ICT Room in order to finalize
the implementation plan of this program. This includes scheduling of every activity to be given
to the learners, checking of their works, recording of scores, preparation of progress charts,
organizing of pupils’ portfolio, the date of post-test assessment, and tabulating gathered data
through pictures and graphs.
In the end of the session, Mr. Isaac S. Balagtas, Jr. a grade 5 teacher and CI Team-
communication review the minutes of meetings signed by CI Team facilitator, and the Team of
Project CREATE.

Narrative Report:

This is the day of testing of those made interventions by CI Team at Grade 5- Quezon
facilitated by the CI team leader, Mrs. Elsie Alceso. And the CI Team.
The session started at 8:00 am and ended at 11:30 am. It is divided into three sessions: The
first activity focused on the orientation about its goals and objectives why this project must be
implemented and the distribution of CREATE portfolio. The second part was the discussion on
how to construct sentence with correct punctuation marks and the rules in subject-verb
agreement, while the last part was showing of pictures of different action words to be interpreted
by the pupils through construction of sentence.
After of this implementation, the team collected all the portfolios of the pupils for checking and
evaluation, basis for improvement and innovation or what ever purpose it may serve.

The CI Team tracking all areas of concern after the testing of interventions were made.
This is to identify if the given innovations were appropriate and aligned to their level of
understanding. If there is need for improvement or changes just to assure that there will be a
development as shown in the progress report of the pupils. Based on the result, most of the pupils
improved in writing and reading skills as we give emphasized on the adoption of Project Spell It
Right (SIR), showing a pictures of different action words, providing learning activity sheets
through construction of sentence involving a subject and verb agreement. It truly evident that we
need to provide more time to practice with the help of educational technology.
The team also make sure that all the data gathered are confidential and to be used only for
educational purposes.

Narrative Report:
. After the careful evaluation of pilot test results, improvement, and changes. The Project
CREATE Team started to implement the said innovation in Grade 5 Quezon class. It lasted from
May 23-28 at 7:30 am-11:30 am. First, we started the session through opening prayer, followed
by singing of the national anthem. After the singing of the national anthem, is the checking of
attendance led all by the pupils.
The second session was the lesson proper; it began by showing a pictures of an action
words spelled by the pupils. If all the words were checked, the team make sure that all pupils
understand the words through unlocking of difficulties. Using their CREATE Portfolio, the
pupils will make a sentence following punctuation marks and subject verb agreement. After of
this session, the team collected all the folders in order to check and record their scores in the
progress report chart.

With supervision of CI Facilitator, Mrs. Elsie Alceso and the members, the team
administered the post-test in grade 5 Quezon class. The assessment started at 8:00am and ended
at 12:00 nn. We make sure that the pupils were properly guided by stating general reminders in
the class including the time allocated, correct spelling, capitalizations, and punctuation marks
that must be observe. When the time is up, the team collected all the test papers for checking
and recording.
