copy his Birth Certificate.

4. I commit a total of $50,000 for his travels.

5. I am presently employed as President and Chief Executive Officer . My company name

is IWA BESU Corporation located in No. 72 Ansons St, Dona Maria Subdivision, Brgy.
Togas, Daraga, Albay ,Philippines. My approximate monthly income is P 200,000.00
($3,000.00. Please also see attached as Exhibit "C" current bank statements from the
Philippine National Bank (PNB) and Banco De Oro Unibank (BDO),Exhibit “D" my
2021 Income Tax Return (ITR)and Exhibit “E" my certificate of Income and
Employment.

6. I undertake to be fully responsible for RENZ PATRICK D. SANTILLAN during his stay in
United States of America and to provide food, accommodation and other necessities including
any medical expenses he may incur during his stay. I will also ensure thot he leaves United
States of America before his Visa expires and if necessary to repatriate him at my own cost.
