Authorization Letter

Dear Sir/Madam,

I, Ms. Ivonn Thea Joyce Olbinar, hereby authorize John Mark Dosejo to
transact on my behalf for the purpose of obtaining a Department of Health License to
Operate (LTO).
Mr. Dosejo is authorized to act as my representative and to perform all necessary
tasks related to the application for the said license. This includes but is not limited to
submitting the application form, paying the necessary fees, and obtaining any required
documents or clearances.

I acknowledge that I will be fully responsible for all actions taken by Mr. Dosejo
in relation to this matter. I also acknowledge that any decisions made, or actions taken by
Mr. Dosejo in connection with this authorization letter will be considered as if I had made
them myself.

This authorization letter is valid until the completion of the transaction or until
further notice from me.
