This is to refer Mrs. Delma Eggleston, who has a tumor, involving stage 2A Lung cancer. She requires
your further treatment and management.

Mrs. Delma Eggleston, presented to the clinic with intense coughing and shortness of breath with
describe pain in the chest radiating to shoulder and back. As well as, a change in her voice with harsh
sound with each breath and change color volume sputum. Subsequently, she was undergone chest x-ray
with a not clear result, she was then proceeded to Ct scan and showed a 5.5 cm size tumor and stage 2A
lung cancer.

Regarding on Mrs. Eggleston medical history, she was diagnosed with COPD last 19 July 2012, she is
currently taking Metformin 5oomg t.d.s for oral hypoglycaemic and Ramipril 5mg daily for hypertension.

For reference, Mrs. Eggleston is currently living with his husband , they are both non-smoker however,
his husband sometimes taking betel quid.

Given the above situation, it would be greatly appreciated, if you could see Mrs. Eggleston upon visit
into your clinic to provide further treatment and management.
