Dear Ms. Piper,

This is to update you regarding Ms. Sarah Keating, who was admitted to our hospital due to Infection of
Skin graft on right forearm. She requires your follow up care upon discharge today.

Upon admission, Ms. Keating was routinely managed by proper wound infection treatment, which
includes of daily wound dressing and IV antibiotics. Currently her wound area was left uncovered and on
monitoring for possible infections.

Given the situation, it would be greatly appreciated if you could provide a daily visit to Ms. Keating
home, to monitor her compliance on ampicillin 250 mg / tab q.i.d until 31 of October, In addition, kindly
monitor her graft site and donor site ( on thigh) for signs of infection and interference.
