Diversified Corporation, after having been duly sworn in accordance with the law, do hereby
depose and say:
1. That Green Agi-Ventures Diversified Corporation occupied a space situated at Lot 5, AMD
Building Cattleya Street, Merville Subdivision, Magdum, City of Tagum Davao del Norte,
Philippines),
2. That I do hereby undertake that the abovementioned business address will serve as a
BOOKING OFFICE ONLY and no stock of goods or materials will be stored in the said area;
3. That I further undertake that should my presentation turn out to be false, I hold the Office of
the City Planning and Development Coordinator of Tagum City, free and faultless from any
liability arising therefrom and assume full responsibility for all damages that may be sustained
by any party, if there is any;
4. That I am executing this affidavit to attest to the truth of the foregoing facts and for whatever
legal purpose this may serve.
