PETITION
COMES NOW petitioner, duly assisted by the undersigned Agrarian Reform Program

Officer (ARPO) II and unto this Honorable Adjudication Board, most respectfully states that:

The petitioner is the Attorney-in-Fact of the title holders, Filipino, of legal age, married

to Roseller S. Ybañez, and a resident of Langatian, Roxas, Zamboanga del Norte where she may

be served with summons and other processes from this Honorable Board, copy of the SPA is

hereto attached and marked as Annex “A” hereof;

Respondent is the Register of Deeds of the province of Zamboanga del Norte who is in

charge in the issuance of certificates of title and with office address at Provincial Capitol

Complex, Dipolog City;

 Pursuant to the provisions of Republic Act No. 6657, the following Transfer Certificates

of Title (TCTs) were issued:

Machine copy of the certificates of title are hereto attached as Annexes “B”, “C”, “D”, “E”, “F”,

and “G” respectively;

Said certificates of title were in my possession but were lost despite diligent efforts to

locate them. Machine copy of my Affidavit of Loss is hereto attached as Annex “H” and made as

an integral part hereof;

Since Transfer Certificates of Title No. T-4044, T-4042, T-4043, T-4045, T-4046 and T-

4047 are beyond recovery, it is respectfully requested that new certificates of title be issued in

the name of Roseller S. Ybañez, Marivic Corpus, Ma. Bonna Corpus, Florencia Alvarez,

Rodrigo L. Badiao, and Eleno Tequiner, respectively.

WHEREFORE, premises considered, it is most respectfully prayed of this Honorable

Board that after due hearing, an Order be issued directing the Register of Deeds of the Province

of Zamboanga del Norte to issue a new owner’s duplicate copy of Transfer Certificates of Title

No. T-4044, T-4042, T-4043, T-4045, T-4046 and T-4047 in favor of Roseller S. Ybañez,

Marivic Corpus, Ma. Bonna Corpus, Florencia Alvarez, Rodrigo L. Badiao, and Eleno Tequiner,

respectively.

own knowledge and/or based on the records on hand;

I attest to the authenticity of the annexes thereof;

I further certify that I have not commenced and/or am not aware of any other action or
proceeding involving the same land, or a portion thereof, or issue in any court, tribunal or quasi-
judicial agency; and to the best of my knowledge, no such action or proceeding is pending in any
court, tribunal, or quasi-judicial agency, and if I should thereafter learn that the same or similar
action or claim has been filed or pending, I shall report that fact within five (5) days from such
notice.
