5 Excellent: Student performs the procedure correctly, and perfectly. Student states
the rationale correctly and completely. Student is able to answer questions
accurately.
4 Very Satisfactory: Student performs the procedure correctly. Student states the
rationale correctly but incompletely. Student is able to answer questions.
3 Satisfactory: Student performs the procedure correctly but failed to states the
rationale. Student is able to answer questions when cued.
2 Fair: Student performs the procedure when cued. Student fail to states the
rationale. Student is able to answer questions when cued.
1 Poor: Student fails to perform the procedure correctly even when cued. Student
does not know the rationale.
Bathing is an essential component of nursing care and a critical time to assess the client.
Types of Bath:
1. Therapeutic
2. Cleaning - routine client care; for personal hygiene
Purpose:
Provide hygienic care to a dependent client confined to bed
Expected Outcome:
1. Client will be cleaned without damage to their skin
2. Client’s privacy will be maintained throughout the procedure
3. Client will participate in their own hygiene as much as possible
4. Client will not feel discomfort
5. The patient will vocalize feeling clean and fresh.

1. Bath towels - 2
2. Wash cloths/Face towel - 4
3. Bath blanket/ Blanket - 2
4. Wash basin - 2
