Criteria For Awards For Club or Organization Achievement
This award is given to a duly recognized club or organization that has created positive impact on the school and/
or community it serve through the implementation of all its planned projects and activities, provided strong support to
the implementation of the school activities and attainment of the school’s objectives, and taken great strides to help its
members develop their potentials.
The candidate shall submit a Portfolio using the areas specified in the criteria.
The table below shows the set of criteria and weights that will be used in the evaluation and deliberation process of this
award. Only those clubs or organizations that have received at least 90% of the criteria shall be awarded.

1.Club/organization performance
a. Plans and develops club/organization’s objectives, projects, and activities
b. Implements projects and activities and delivers services based on the club/organization’s objectives and 50%
plans
c. Manages and/or uses resources wisely
d. Use team work and collaboration among its members.
2. Exemplary Output 30%
Delivers a concrete output related to the objectives and purpose of the organization and the school.
3. Contribution to the school or community
Benefits the members of the club/organization and the greater majority of the school population and/or
community 20%
Note: Peer evaluation is only applicable to disciplines that involves collaborative work. Candidates will be evaluated by
at least 30% of their peers ( group, team, class, or club mates) as well as their class or club adviser.
RUBRICS:
Point system shall be used in the computation of the learners’ performance in specific disciplines. 10 being the
highest (100%)
1. Club/Organization Performance
Club/organization performance shall be based on the final rating or average. The procedure
for determining the performance points is given below:
1.2 Implements projects and activities and delivers services based on the club/organization’s objectives and
plans.
1.1.1 Mode of verification: attendance sheets, communications, programs, permits certificates and
pictures.
1.2.1 Multiply the score by 0.15
2. Exemplary Output
2.1 Mode of verification to this criterion will be accomplishment reports with pictures
and captions with the following format: What, Where, When, Why and How.
2.2 Gained points will be multiplied by 0.30.
The candidate aspiring for recognition shall present to the committee evidentiary documents of his/her
contributions to the school and/or community such as:
