CRITERIA FOR AWARDS FOR CLUB OR ORGANIZATION ACHIEVEMENT

This award is given to a duly recognized club or organization that has created positive impact on the school and/
or community it serve through the implementation of all its planned projects and activities, provided strong support to
the implementation of the school activities and attainment of the school’s objectives, and taken great strides to help its
members develop their potentials.

The candidate shall submit a Portfolio using the areas specified in the criteria.

The table below shows the set of criteria and weights that will be used in the evaluation and deliberation process of this
award. Only those clubs or organizations that have received at least 90% of the criteria shall be awarded.

Note: Peer evaluation is only applicable to disciplines that involves collaborative work. Candidates will be evaluated by
at least 30% of their peers ( group, team, class, or club mates) as well as their class or club adviser.

RUBRICS:

Point system shall be used in the computation of the learners’ performance in specific disciplines. 10 being the
highest (100%)

1. Club/Organization Performance
Club/organization performance shall be based on the final rating or average. The procedure
for determining the performance points is given below:

1.1 Plans and develops club/organization’s objectives, projects, and activities


1.1.1 Mode of verification: action plans and project designs.
1.1.2 Add the scores by area.
1.1.3 Multiply the total score by 0.13.

1.2 Implements projects and activities and delivers services based on the club/organization’s objectives and
plans.
1.1.1 Mode of verification: attendance sheets, communications, programs, permits certificates and
pictures.
1.2.1 Multiply the score by 0.15

1.3 Manages and/or uses resources wisely


1.1.1 Mode of verification: budget proposals and liquidation of expenses with receipts.
1.3.1 Multiply the score by 0.10.

1.4 Use team work and collaboration among its members.


1.4.1 Mode of verification: peer and club adviser’s evaluation.
1.4.2 The weight for this criterion will be 5% from the peer evaluation and 7% from the club adviser.
1.4.3 The total score from peer evaluation will be multiplied by 0.05 then for the club adviser’s
evaluation result will be multiplied by 0.07.
1.4.4 Add the peer evaluation result and the adviser’s evaluation result.
1.4.5 The evaluation tool below will be used by the club/organization members and advisers in rating.
