AFFIDAVIT OF ATTACHMENT OF SIDE CAR

 I, DANTE P. RAMOS, of legal age, Filipino, married, and a resident of Barangay
Navitas, Catanauan, Quezon Province, after having been duly sworn to in accordance
with law, depose and state that:

1.  I am the registered owner of a motor vehicle particularly described as

follows:

Certificate of Registration No. 442960674
Denomination: MOTORCYCLE WITHOUT SIDE CAR

2. The original body type or denomination of the said motor vehicle is:
"Motorcycle without Side Car";

3.  I have decided to attach a sidecar to the motorcycle unit, which

attachment was not included in the registration. In view of this, a change of the body
type of the said vehicle from "Motorcycle without side car" to "with side car" is
appropriate;

4.  The change of body type is not intended for any illegal or unlawful
purpose; and

5.  I am executing this Affidavit to change the body type of the said
motorcycle from motorcycle without side car and to attest to the truth of the foregoing
and in support of my application for the change of body type of the above-described
motor vehicle on its Certificate of Registration.
