    I,  KOTADA RAMANAMMA W/O KOTADA APPARAO HINDU  aged 52 years


    AADHAR:934569810364) residing at door no. 13/170, Donkada, Aganampudi, Near Ramalayam,
    Aganampudi, Visakhapatnam- 530046, Andhra Pradesh do hereby solemnly affirm and state as follows.

      I submit that I am a  labourer and my annual income is Rs. 84,000/- (rupees Eighty four thousand
    only) I submit that the Tahsildar, Gajuwaka issued rice card Bearing No2819009422 as I am poor. I submit
    that nobody is government employee  in our family and I have no other sources of income. 

    I request the Tahsildar, Gajuwaka Mandal, to issue Income Certificate, in favor of me for getting
    benefits from govt., of Andhra Pradesh i.e., YSR CHEYUTA.

     DEPONENT

    Solemnly affirmed and signed before me on this 22nd August, 2022 at Aganampudi, Gajuwaka
    Visakhapatnam.
