I, JOHN CARLO C. FERNANDEZ, Filipino, of legal age, single, and resident at

Barangay Tumaga, Porcentro Comet St, after having been duly sworn on oath in
accordance with law hereby voluntarily deposes and says;

1. That I am an applicant for Special Enlistment with the Philippine Army;

2. That one of the qualifications for Special Enlistment is that one must be single
and has never been married before;

3. That through this affidavit I would like to declare under oath that I am still
single and has never been married before nor had any issue or child out of
wedlock;

4. That I am executing this affidavit to attest to the truth of the aforementioned

facts and to support my Special Enlistment with the Philippine Army.
