JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, ESTERIA A. GAVIOLA, and ANALYN C. GENSON, of legal ages, both

Filipinos and residents of Vallehermoso, Carmen, Bohol, Philippines, after having been duly
sworn to in accordance with law, do hereby depose and say:

1. That we personally know the person of FAMILLA GARAN OYANGORIN, for we are
friends thereof;

2. That as per FAMILLA GARAN OYANGORIN’s Certificate of Live Birth issued by

the Philippine Statistics Authority (PSA), her true and correct name is spelled as
FAMILLA GARAN OYANGORIN;

3. That as per FAMILLA GARAN OYANGORIN’s Personal Record/Unified Multi-

Purpose ID (UMID) Card Application issued by the Social Security System (SSS), her
name was incorrectly spelled and recorded as FAMILLA OYANGORIN;

4. That the name FAMILLA GARAN OYANGORIN as per her Certificate of Live Birth
issued by the Philippine Statistics Authority (PSA), and FAMILLA OYANGORIN as per
her Personal Record/Unified Multi-Purpose ID (UMID) Card Application issued by the
Social Security System (SSS), refers to one and the same person living and existing in
our community;

5. That we are not related to the said person either by affinity or consanguinity and that we
have no interest in whatever outcome this presents may bring;

6. That we execute this affidavit voluntarily to attest to the truth of the foregoing statements
for whatever legal purpose this may serve.
