I, MANIE D. CASUNCAD married, Filipino, of legal age, and a
resident of Tigum, Buruanga, Aklan, after being duly sworn to
in accordance with law, hereby depose and state:

1. That to this date, I am not employed in any capacity at

any corporation, company, individual person, or with the
government;
2. That I have applied to avail the benefits of being a SOLO
PARENT since I was left alone with the responsibility of
taking good care and providing support for my children;
3. That I am executing this Affidavit of Non-Employment to
attest to the truth and veracity of the foregoing and for
the purpose of securing a Solo Parent ID and to receive
the benefits under the Solo Parent’s Welfare Act of 2000.
