Welcome! Come join DMAE Student Council to dance the night away at our Winter Wonderland Formal. FOR THE FIRST
TIME EVER, WE ARE HOSTING AN EVENT AT A VENUE! Come join in on the fun!

This is the first form you must complete by FEBRUARY 14th at 11:59 PM.
You must also print out a second form PHYSICAL PAPER WAIVER THAT YOU MUST BRING TO THE DANCE

What to expect from the dance!

MUST BRING TO THE DANCE:

- SCHOOL ID WITH CURRENT SCHOOL YEAR
- IN-DISTRICT AND OUT-OF- DISTRICT STUDENTS MUST BRING A SIGNED PAPER WAIVER TO THE
DANCE OR HAVE HANDED IT IN WITH MS.OSHEA BEFOREHAND
If the waiver is not handed in, then you will not be able to enter the dance.

1. Students attending the dance have TO BE A CURRENT HIGH SCHOOL STUDENT
2. NO alcoholic beverages or any substances ALLOWED( (this dance is a substance free dance!)
3. No large bags or backpacks!
4. A DMAE Student may invite any number of Out-of-District Guests

Dress Attire:
Wear Semi Formal Attire!
NO JEANS, SLIDES, PAJAMAS, ETC.

Reminder: A Non-Dwight Morrow/Academies @ Englewood attendee needs to be invited by a DM/AE student.

With that, the DM/AE Student will hold full responsibility for the guest until the end of the dance.
