JOINT AFFIDAVIT

We, RENANTE Z. REALISTA and GREMELY Q. SOBREVIÑAS,

Filipino, of legal age, with address at Dumaguete City, Negros Oriental, after
having been sworn in accordance with law, state that:

1. We have known ROSENDO BERGANIO TUBOG for several years

now;

3. In his Certificate of Live Birth, her name was inadvertently spelled

as ROMEO BERGANIO TUBOG, which is clearly an erroneous entry;

4. He has been using this name ever since and even his Identification
Cards would show his name as ROSENDO BERGANIO TUBOG;

5. We hereby attest that the person who appears with the name
ROSENDO BERGANIO TUBOG and ROMEO BERGANIO TUBOG is one and
the same person;

8. We execute the foregoing affidavit in order to attest to the

truthfulness and the veracity hereof, and for all other legal intents and purposes;
