PROCEDURE FOR THE APPLICATION


OF BUSINESS PERMIT

I. Application of Permits and Licenses

1. Barangay Clearance for Business

1.1 Application requirements

1.1.1 New Application

1.1.1.1 The following shall be prepared before submission to the Barangay Office
for Clearance Certificate:

1.1.1.1.1Signed and sealed Blue Print of all Building Plans


1.1.1.1.2For renovation/retrofitting, a copy of the Contract of Lease if
Building is rented
1.1.1.1.3Letter of Intent

1.1.2 Renewal Application

1.1.2.1 The following shall be prepared before submission to the Barangay Office
for Clearance Certificate:

1.1.2.1.1A copy of the previous Barangay Permit Clearance


1.1.2.1.2A copy of the Official receipt of Barangay Permit Clearance used
as support for the application

1.2 Processing

1.2.1 All required documents shall be submitted for the application of Barangay Permit
Clearance to the Barangay Office

1.2.2 Payment of corresponding amount for the assessed value on said clearance
certificate shall be settled accordingly

1.2.3 Clearance shall be picked up after processing and release on the same day upon
application

2. Business Permit

2.1 Application requirements

2.1.1 New Application

2.1.1.1 A Business Permit Application Form shall be accomplished and


notarized

2.1.1.2 The following documents shall be included in the submission of
accomplished and notarized application form to the Business Permits
and Licensing Office (BPLO)

2.1.1.2.1Business Name Registration from the Securities and


Exchange Commission (SEC)
2.1.1.2.2Community Tax Certificate
2.1.1.2.3Barangay Clearance for Business
2.1.1.2.4Certificate of Occupancy
2.1.1.2.5A copy of the Lease of Contract if area or place of
business is not owned by AG&P
2.1.1.2.6Secretary Certificate for Capitalization
2.1.1.2.7Fire Safety Clearance from the Bureau of Fire Protection
(BFP)

2.1.2 Renewal

2.1.2.1 Renewed Barangay Permit Clearance

2.1.2.2 Both original and copy of the Bureau of Internal Revenue (BIR)
monthly/quarterly payments from the previous year

2.1.2.3 Financial Statement covering the past two (2) years duly filed before
the SEC Office with Certification of No Derogatory Information

2.1.2.4 A Copy of the Mayor’s Permit covering the previous year together with
its corresponding assessment and validated Official Receipts

2.1.2.5 Latest Community Tax Certificate of the present year

2.1.2.6 Fire Safety Clearance from the BFP

2.1.2.7 Other additional documents deemed necessary by the BPLO where


application is lodged.

2.2 Processing

2.2.1 BPLO Application Form shall be accomplished and submitted together with all the
application requirements to BPLO office

2.2.2 Payment of the assessed value for the application shall be made at the City or
Municipal Treasurers Office

2.2.3 Official Receipt of payment from the City or Municipal Treasurers Office shall be
presented to the BPLO for acknowledgement that application has been paid

2.2.4 At least one (1) week shall be allocated before lodging follow-ups with BPLO for the
release of Business Permit Renewal
