REPRESENTATIVE FOR AND IN BEHALF OF THE ASSOCIATION FROM THE OFFICE OF <name of
Regional Director, REGIONAL EXECUTIVE DIRECTOR OF THE DEPARTMENT OF AGRICULTURE-
REGIONAL FIELD OFFICE III IN THE IMPLEMENTATION RICE COMPETITIVENESS
ENHANCEMENT FUND (RCEF) OR RICE FUND.

WHEREAS; the NAME OF FCA, with address at Brgy. Address that having control over all aspects of the palay
and rice industry is the only way to attain its dream of genuine agricultural development;

WHEREAS; the Association has been successful in yielding socio-political and economic gains in its small-scale
palay production;

WHEREAS; to improve its productivity and providing mechanized farming service to its farmer members at a
minimum cost, the association needed to acquire any Rice Competitiveness Enhancement Fund (RCEF) component
or Rice Fund;

WHEREAS; the Association has a total of AREA hectares of irrigated rice land and AREA hectares of non-irrigated
rice land which is more than or equal to the minimum required area of AREA hectares of irrigated/non-irrigated rice
land to avail assistance of any Rice Competitiveness Enhancement Fund (RCEF) component or Rice Fund;

WHEREAS; the Association has an average palay yield of YIELD metric tons per hectare per year;

WHEREAS; the NAME OF FCA has NUMBER farmer members that will be directly benefit under the project and
has access to the services of any Rice Competitiveness Enhancement Fund (RCEF) component or Rice Fund;

NOW THEREFORE; on motion of NAME and duly seconded by all board of directors present, it is hereby
RESOLVED that the NAME OF FCA will authorize NAME OF CHAIRMAN as representative for and in behalf of
the Association from the office of MR. CRISPULO G. BAUTISTA, JR., OIC-Regional Executive Director-
Regional Field Office III in the implementation of Rice Competitiveness Enhancement Fund (RCEF) or Rice Fund;

RESOLVED FINALLY, that NAME OF CHAIRMAN that this copy of the request shall be forwarded to the
concerned agencies.

RESOLVED FINALLY, that NAME OF CHAIRMAN Chairman of the NAME OF FCA and as the authorized
representative to transact business with the DA and/or sign documents for and in behalf of the Association with his
three (3) specimen signatures shown below this resolution.
