Assessment:

Subjective: Patient says she’s having headache and nape pain.

Objective:
VS: BP 180/100 for 3 takes with 15 minutes intervals,
RR 16 cpm, CR 100bpm.
The patient has a maintenance drug of valsartan 80 mg once a day but with poor compliance
with medication and diet management.

Diagnosis:
Ineffective Health Maintenance Behaviors and Pain - related to ineffective maintenance of
medication and poor lifestyle as evidenced by the patient + poor compliance with medication
and diet management + abnormal bp findings + headache and nape pain.

Planning:
- The patient will report decreased pain within 5 hours.
- The patient will report normalized bp within 5-7 hours
- The patient will learn how to maintain her medication and lifestyle within 1-2 hours after
the patient feels better.
