JOINT AFFIDAVIT OF TWO (2) DISINTERESTED PERSONS

WE, NELIA OBEDA CLAUD and CONCHITA A. MELGAR, both of legal ages,
Filipinos, and residing at B38 L3 San Isidro Labrador I, DBB, Dasmarinas City, Cavite, and B40
L18 San Isidro, Labrador I, Dasmarinas City, Cavite after having been duly sworn to in
accordance with law hereby deposes and say:

That we personally know PABLITO BOLORON ARADO, who was born on October
18, 1970, and currently residing at Blk 38 Lot 3, Brgy. San Isidro, Labrador I, DBB,
Dasmarinas City, Cavite;

That we know for a fact that since birth he’s been using his name as “PABLITO
BOLORON ARADO” and his birthday “October 20, 1970”, thus, it reflects in his
government identification cards and other documents

However, when he secures a copy of Certificate of Live birth by National Statistic Office,
it appears his name is “PABLITO BOTORON ARADO” and his Birthday is
“OCTOBER 18, 1970”, although her maiden’s mother’s surname is “BOLORON”,
which it appears that there is a discrepancy in his middle name;

That we hereby and testify that his correct and true name and birthday is PABLITO
BOLORON ARADO born on “OCTOBER 18, 1970” based on our personal knowledge
and Certificate of Live Birth

That “PABLITO BOLORON ARADO born on OCTOBER 20, 1970” and

“PABLITO BOTORON ARADO born on OCTOBER 18, 1970” and “PABLITO
BOLORON ARADO born on OCTOBER 18, 1970”, as described herein, are refers to
one and the same persons;

This we are affidavit is executed for the purpose of attesting our personal knowledge of
and for all legal intents and purposes;
