HOW TO USE THIS FORM
1 At the start of the year :-
a) Weight the important factors for the Employee to achieve (Fill Part 1 ; Column A).
b) Agree on KPIs, how they will be measured (Fill Part 1 ; Column B, C and D).
c) A discussion between Appraiser and Employee should be conducted for better understanding
of targets set. A copy of this form (set KPIs) should be given to the Employee.
2 At the end of the period :-
a) Employee completes "This Year's Actual" (i.e. Part 1, Column E)
b) Appraiser completes the rest of the form (i.e. Part 1, Column F & the entire of Part 2)
c) Employee's Score is auto-calculated (i.e. in Part 1, Column G & Part 2, last row) by Excel.
100% TOTAL SCORE - PART 1 * 3 points if this year's target is achieved
d) Next level of Management must confirm the Assessment.
