This is to hereby certify that Dr. USMAIMA F. USMAN, Medical Officer III and
designated as the University Physician/Medical Director of the Medical Services, MSU-
General Santos City has provided an outstanding performance/service to our constituents
during the state of Public Health Emergency brought about by the Corona Virus Disease in 2019
(COVID-19)
As such, other than taking the lead in all the university actions/intervention in the
protection and promotion of the health and wellness condition of the students, faculty and staff
including their family dependents, she has also satisfactorily attended and managed patients
brought to the university infirmary. Furthermore, Dr. Usman has established an effective
referral-relationship with other hospitals in the city of patients needing further medical attention
and diagnosis.
On the other hand, Dr. Usman has also built and strengthened partnership,
collaborative and coordinative efforts with the Barangay Fatima Rural Health, General Santos
City in addressing medical-health needs of the barangay constituents much more in the series
holding of Covid-19 Vaccinations. In fact, her performance allowed her to be officially known as
School Health Consultant I by the Philippine Academy of Physician School Health
Incorporated (PAPSHI).
Finally, Dr. Usman is closely coordinating with the Department of Health (DOH)
and the Commission on Higher Education (CHED) for the compliance of requirement and rules
for the final accreditation of the University Infirmary.
Issued this certification to the above-mentioned party to support her claim for
compliance of the continuing Professional Development requirement and application, and for
