Re. Mrs Constance Markwell

I am writing to refer Mrs Markwell, a 72 year old married mother of 3 adult


children who is presenting with a visual impairment.

Initially, she presented to me on 21/2/06, complaining of inflamed, sticky


and weeping eyes. Both her eyes were reddish with watery discharge.
However, her right eye was worse than the left eye. Therefore she was
prescribed chlorisig drops 4 hourly. She has had thyroidism for 3 years, high
blood pressure for 1 year and a hip replacement was done in 2005. Her
current medications are Thyroxin 1 mg, Atacand 4 mg and Fosamax 10 mg
daily. She has no known allergies.

On review after 2 weeks, she had made no improvement. In addition she


had blurred vision with oedematous eye lids and conjunctival conjestion., so
chloramphenicol was prescribed 0.5% one drop three times daily and Bion
tears one drop 4 hourly.

Unfortunately, today she was accompanied by her husband with complaints


of impaired vision in both eyes and an inability to read books or watch
television. There was oedema in both eyelids with white discharge. She
could not read the eye chart.

In view of the above signs and symptoms I believe she needs immediate eye
care facilities. I would appreciate your urgent attention to her condition.
