5811 JACOBO STREET, BRGY. POBLACION, MAKATI CITY for the purpose of
pest control services. For the purpose I hereby certify that Iam waiving any claim from MAPECON
PHILS., INC. for any liability for damage/s that may occur during the process of drilling due to:

Our non-disclosure or inability to provide any underground water/electrical installation plan of

my house/warehouse/building as resquested and required.

Drilling location plan has been mtually agreed on, as attached and initialed.
(This waiver is deemed void if when actual drilling diverts from agreed plan)

I voluntary allow MAPECON PHILS., INC. to proceed with the drilling service as deemed necessary.
