1. SCOPE OF WORK

Location ASSA North, Imo State


Total Perimeter fence distance 2850.56m
Pole description 6m high, 4” Galvanized
Distance between poles 40 meters
Light Fitting 80W Solar All-in-one Streetlight
Concrete base description Please see Engineering design document

SN TASK STEP TASK DESCRIPTION

01 Off-site and Pre-Fabrication works o Fabricate concrete base offsite in line with project
specification.
o Weld base plates to Streetlight poles
o Complete all painting works
02 Site Preparation Removal of obstacle from planned installation area including, trees,
grass, etc.

03 Mobilize to site Manually move work tools and materials, this includes:

 Personnel

 Work tools and materials

 PPEs

 Extension wires and cables


04 Civil Works Digging and installation of concrete base for solar poles

 Digging of base 2feet or less

 Installation of Precast Ballasts

05 Installation of streetlight poles and Mounting and erection of streetlight poles


Solar All-in-one Light fittings
 Coupling of All-in-one Solar Lamps to Iron poles

 Fastening of Poles with base plates to concrete base

 Confirm Erected poles with Solar lamps is well aligned


06 Electrical works  Construct and install central earthing system

 Run earthing cable chain to all installed streetlight poles

 Dressing of cables and piping


07 Evacuate Waste Generated Cart away all waste Generated from site and tidy up site properly
before moving out from site.

08 Housekeeping Ensure all work areas are clear of sharp objects and particles

09 Testing and Commissioning Confirm all Led lamps are working fine and Handover

Estimated project duration is 13 working days. However, due to current measures set-out by
the Federal Government to combat the spread of COVID-19 Virus and the security situation
of the terrain, delivery time maybe longer due to market restrictions and curfew

A total of 6 personnel will be required to execute this project. 4 staff from Auxano team
and 2persons from the local community.

4. TRANSPORTATION AND ACCOMODATION PLAN

From our experience in previous projects within same community, we plan to have a mix of
our project team and local workforce for the execution of this project.

5. HSE PLAN

HSE Objectives

The objectives however are to comply with Shell HSSE and contract HSSE
requirements and prevent incidents while this activity is being executed.
o No harm to people, environment, and no leaks.
o Has a systematic approach to HSSE management designed to ensure compliance with
the law and to achieve continuous performance improvement
o Sets targets for improvement and measures, appraises, and reports performance.
o Engage effectively with neighbours and impacted communities
o Ensuring that hazards and risks associated the project are identified and managed to
ALARP.
o Educating the workers and public to the benefits of good house keeping o Ensuring
that staff competence is improved geared towards effective service delivery. o
Ensuring that personnel are issued with the right tools and equipment to execute jobs
safely.
o Set Key Performance Indicators (KPI) that will aid achieving Goal Zero.

HSE Target

Daily Toolbox Talk shall be conducted before commencement of any low,


medium, high or extreme risk job task. This will ensure that workers become aware
of all aspects of the task, HSE implications and the role they are to play should
problem occur.

▪ Ensuring that all personnel intervene in unsafe acts/conditions and timely


reporting.
▪ Daily Inspection of worksite shall be done to identify any inherent hazard
and barrier put in place.
▪ 100% Compliance with Work Instructions and Procedures and there shall be
no deviations.
▪ Daily Discussion of Unsafe Act/Condition Prior to Start of Work. Ensuring
that all personnel intervene in unsafe acts/conditions and timely reporting.
All UA/UC/NM shall be recorded for proper close out.
▪ Ensure all Life Saving Rules is observed for the duration of the project.
6. OUR REPONSIBILITIES

Responsibilities shall include:

• Implementing the company HSE plan for the contract


• Provide field construction management and supervision with the necessary services
relative to quality activities.
• Project delivery within the agreed time frame/work schedule.
• To deliver the project in line with the agreed scope of work and drawings.
• To ensure project delivery without compromising the health of the workforce.
• Ensure that the appropriate permits are obtained before work commence.
• Carry out thorough analysis of statistical data and inspection, make recommendation for
solution
• Liaising with Site Manager and or Managing Director to promote HSE on site.
• Promoting employee participation in HSE matters
• Implement safety rules and regulations
• Investigation of all injuries and illness
• Conducting of site HSE meetings and coordination of tool-box meetings /TRIC
implementation.

• Conduct all took box meetings, site HSE meeting and induction courses for new
employees.

• Liaise with Oilservltd (Terminal) HSE coordinator for update


• Ensure that the entire workforce respond to emergencies.

6.1. MECHANICAL FITTER

• The fitter oversees the crew

• 9. EMERGENCY PLAN

In the event of any minor fire incident, it would be put out using portable fire extinguishers, while major
fire incidents require more technical approach and trained fire fighters shall be invited to fight the fire.
In case of fire, workers are advised to act as follows:
Raise alarm to warn people in the vicinity
• Shut down power supply and remove any combustible materials near the fire, close doors
and window, but do not lock
• Fight fire with appropriate fire equipment if it is safe to do so
• Inform supervisor immediately
• Leave scene of incident and gather at the muster point at the sound of the alarm for roll
call
• Do not go back to work until you are told to do so.

All workers are to know the proper exits, corridors, and responsibilities of their section.

Each worker should know where to report upon evacuation and should stay there until roll call is taken
or a supervisory authority gives further instructions.

• 10. GENERAL SITE HSE REQUIREMENTS

All staffs of AUXANO SOLAR NIG. LTD. shall have to adhere strictly to all HSE rules and
procedures on site. Among these rules are
- Risk assessment and toolbox talk shall be conducted before commencement of any
medium, high or extreme risk job task. This will ensure that workers become aware
of all aspects of the task, HSE implications and the role they are to play should
problem occur.
- Use PPEs provided for the task at hand.
- Obey all safety signs. No unauthorized persons shall have access into the work area.
- Obey Life Saving Rules.
- Observe and record UA/UC/NM. Monitor to ensure close out of items.
- Keep site clean and waste-bins. Waste shall be segregated properly.

• Stop work authority:


All the workers have the authority to stop work when it is discovered not to be safe. Safety
is the key priority and everyone’s responsibility.
