INFORMED CONSENT

Greetings of Peace!

We are students from Yllana Bay View College studying Bachelor of Science in
Criminology. We are now on the process of completing our research entitled “THE
FAMILY PROTECTIVE-MEASURES AND ITS EFFECTS IN PREVENTING ON
JUVENILE DELINQUENCY IN BARANGAY SEBUCAO DUMALINAO”.

With this in mind, we need the participation and cooperation of some of your
local children to be the part our research.

In this matter, we are respectfully seeking your voluntary participation for

being qualified to give your informed consent to take part in this study. Before you
decide whether to participate or not in this study, please read the succeeding
information about the study and feel free to ask questions anytime should there be
anything you do not understand or want to clarify. If you agree to answer this
questionnaire you will be asked to affix your name and signature on this form for
which you will be given a copy.

The purpose of this study is to find out the significant relationships between
the family based-protective measures and its effects in preventing on juvenile
delinquency.
