1.PURPOSE
This procedure establishes the individual and organizational responsibilities of KJO personnel
involved in the emergency evacuation to successfully evacuate a sick or injured person to the KJO
Hospital Emergency Room (ER).

2.SCOPE
This document applies to all cases within the jurisdiction of Al-Khafji Joint Operations requiring
medical response and evacuation. This will also describe the sequence of steps to follow when
evacuating sick or injured personnel from the work site, both onshore & offshore.

3.DEFINITION
KJO: Al-Khafji Joint Operations

MEDEVAC: Medical Evacuation

KJO Hospital: Company hospital owned and managed by Al-Khafji Joint Operations for the
provision of medical services for its employees working in Al-Khafji Joint
Operations.

Medical Facility: KJO plant medical facilities both onshore (south area) & offshore available to
provide first-aid medical services for the patient/injured personnel as
necessary.

ERT: Emergency Response Team; An organized group of employees within Al-Khafji


Joint Operations plant areas who are trained to work together and support the
company firefighters in response to on-site emergency incidents.

4.ROLES AND RESPONSIBILITIES


4.1 Industrial Security & Safety Department (ISD)
Ensure evacuation route is viable and provide as necessary a non-medical (security) escort
and advise the accompanying crew of any possible threats or risks.
If deemed necessary, consult local government officials.
Conduct MEDEVAC drills at least twice per year to test internal response time of the
Emergency Response Team (ERT) and Medical team.
Ensure that all key personnel are oriented in the implementation of this procedure.

Ensure Contacts are in place with KJO Emergency Response Team (ERT), Government
Authorities, hotlines and radio communications are available as per KJO Emergency
Response Plan.
Ensure adequate personnel (Emergency Response Team) are available and are authorized
to mobilize the MEDEVAC as necessary.
In all cases of MEDEVAC, all arrangements for the patient(s) at their final destination shall
be carried out by KJO Hospital staff, including ambulance pick-up and medical care.
Alert all emergency response team and ensure availability of firefighters at the
heliport/helideck as necessary.
Update the information and evaluate the effectiveness of this procedure and initiate a
review in consultation with the Medical Services Department and Emergency Response
Team Members (ERT).

4.2 Medical Services Department (MSD)


Manage the medical team for the preparation and implementation of the MEDEVAC actions.
Ensure that Medical Staff are equipped, trained and capable of handling all medical
emergencies and MEDEVAC as necessary.
If required, arrange out-of-Khafji medical facilities of appropriate capacity and quality of
service to accommodate cases beyond the control of KJO Hospital, for Emergency Medical
care of patients requiring this care as per the MSD contingency plan for disasters and
approved DMFs (designated medical facilities).
Once the MEDEVAC is decided, MSD shall be responsible to notify the KJO Hospital
management and liaise with the medical staff for all MEDEVAC requirements. Also to
establish contact with on site medical personnel to brief and follow up the receiving doctor
on the medical condition, needs and treatment.
Responsible for putting in place MEDEVAC logistics including alerting KJO Medical staff
and ensures that doctor travels offshore if necessary, or Doctor consult with the site
medical staff by radio or telephone, and that patients are received by ambulance and
admitted to hospital if necessary.

4.3 Worksite Site Medical Personnel


Ensure that the Medical facilities (offshore and South area clinics) are equipped with the
equipment and consumables to accommodate trauma and emergency cases and capable
of stabilizing patients and sustaining life.
Be the ultimate authority on the site medical facility (onshore & offshore) to take medical
decisions independent of administration issues.

Responsible for determining the MEDEVAC Priority and advising the concerned
supervision personnel accordingly and ensure to attend every accident, casualty and every
case of illness to apply treatment as necessary.
Ensure to administer all medical treatments and preparations necessary to successfully
MEDEVAC patients to the KJO hospital emergency facility or destination medical center.

4.4 Manager, Terminal & Marine Department (MTM)


Responsible to ensure that marine and/or air transportation requirements are managed and
successful evacuation of patients through sea or air is implemented at all times.
Liaise with the aviation staff for the immediate availability of pilots where air transportation
is urgently required.
Captains and pilots will determine any safety or operational issues concerning seaborne &
flight conditions, day or night.

4.5 Area Authority / Worksite Supervisor


Provide necessary assistance to medical staff which include but not limited to contacting
the concerned support group to organize the Sea or air transportation requirements and
Ambulance transport to KJO Hospital emergency facilities in accordance with the
company’s Emergency Response Plan.
Arrange for all documentation and official papers necessary for the MEDEVAC other than
the medical records and ensure that all necessary forms and reports are filled out and
documented. Copies of these reports to be sent to the ISD Manager / ERD Manager / MSD
Manager / Proponent Department Manager as per the Incident Investigation and Reporting
procedure No. KJO - 6406.
Liaise with the concerned department supervision personnel on all matters related to
MEDEVAC and in establishing contacts with the emergency response team and shall
consult with the KJO Hospital Doctor for Medical advice.
Report all major medical cases and MEDEVAC cases to the ISD Manager / ERD Manager /
MSD Manager / Proponent Department Manager immediately.
Assist the Medical Team in the MEDEVAC as necessary.

5.PROCEDURE
The KEY to a successful and efficient MEDEVAC is to maintain COMPLETE CONTROL of the
patient medically, and the case in general at all times. Medical Evacuation is a case-by-case
scenario, however, all medical evacuation options will be evaluated and an assessment made for
suitability of medical evacuation based on the below mentioned classifications.

Patients will be MEDEVAC based on a medical decision taking into consideration their overall
medical condition and medical need. Accordingly, medical cases will be classified as detailed
below.

5.1 Classification of Medical Cases Requiring MEDEVAC


Significant work related illnesses or injuries affecting employee(s) will require the
employee(s) to be evacuated to KJO Hospital for further expert investigation or
management. Employee(s) may be MEDEVAC from their location(s) to a suitable Medical
facility based on Medical Condition / Classification and availability of Medical Services /
Centers to meet the medical needs of the patient(s). Medical evacuation is classified into
the following categories:
Non-urgent / non- emergency:
These are cases that require further expert investigation and/or medical management are
not available at the incident site. These cases will be evacuated routinely to the KJO
hospital emergency facility.

Urgent / Emergency:
The urgent or emergency cases require medical expertise not available on the incident site
medical facilities but the urgency of providing suitable medical care is required. Decision
to MEDEVAC depends on the nature of the medical condition and the proximity of the
location of the incident site. The key is to maintain the patient’s medical condition stable
and to guarantee early and prompt resuscitations of a rapidly deteriorating Case.

Emergency / Critical:
When the case is time-critical, a quick response with an appropriate medical team in
addition to medical supplies and equipment and route of evacuation are critical in
succeeding with such a medical evacuation. These are necessary for the safe and timely
evacuation of sick-injured patient.

5.2 Emergency Notification


In the first instance and as per the Emergency Response Plan, notification of the emergency to
the worksite medical facility and/or KJO Hospital emergency medical facility should include:
Name(s) and exact location of the patient(s)
Nature and gravity of the illness or injury
The time of the accident,
Employer of the person(s),
Whether the patient is conscious or unconscious,

Whether the patient is breathing freely or not,


Identity of the caller, his location, extension number and mobile number (if outside plant /
office).
Whether or not a Doctor is required to accompany the patient(s)

5.3 Primary Medical Care


In case an individual sustains an injury, first aid and emergency medical care shall be
administered by the first aiders and/or nurse or paramedic as specific to the location of the
individual.
Based on the severity of the injury or illness and in some cases the patient sustaining a
more serious injury or illness will require more sophisticated medical care and
investigation/examination, the individual is then transferred to the KJO Hospital
Emergency Room where appropriate medical care is administered.

5.4 Medical Decision to MEDEVAC


Patient(s) whose medical condition and medical need require treatment on medical care
services that are not available in the on site medical facilities, will be MEDEVAC to KJO
Hospital Emergency Room to attend to the needs and treatment of the patient(s).
Based on all the variables in assessing a patient’s condition and needs, the decision to
MEDEVAC will be taken.
The Mode of MEDEVAC will be determined based on the assessment of all viable options to
MEDEVAC within KJO onshore & offshore operations and/or out-of-khafji. See MEDEVAC

5.5 Mode of Evacuation
There are several possible options for a MEDEVAC of a patient. The possible options will be
described; however the final mode of MEDEVAC will be decided based on medical conditions,
medical need, and destination as these factors highly influence the choice.

5.5.1 Transport by Road:


Ordinary Car/Vehicle could be able to transport some conditions where emergency and
trauma medical equipment is not needed.

Ambulance is fully equipped with emergency and trauma case equipment and
consumables and should be selected to transport Emergency and serious Medical
Cases.

5.5.2 Transport by Air


Air evacuation is one of the preferred methods of evacuating patients due to the fact that
it is more advantageous for facilitating speedy delivery of the patients.

5.5.3 Transport by Sea


Sea evacuation is the alternate method of evacuating patients when transport by air is
not available, specifically for offshore-based employees. Rough weather condition is one
of the factors that affect the availability of a helicopter. Mode for sea medical evacuation
could be through the following options:
 Standby crew boat at the Gathering Station.
 Contractor Supply boat
 Security Patrol boat

5.6 Preparing the vehicle / vessel / aircraft for MEDEVAC


Based on the selection of mode of evacuation, the attending nurse/doctor has to ensure all
necessary medical equipment and supplies are fitted in the vehicle / vessel / air craft.
In the cases where the carrier is an ambulance or an aircraft already fitted with emergency
medical equipment, a final assessment (checklist) needs to be made to ensure all
equipment and supplies are available.
Additional measures to be taken might include Litter straps, clamps, flashlights, ear plugs,
loose items secured, communication, and security escort / admin escort if necessary.

5.7 Medical Service Management


Once a patient (KJO or Contractor) is transferred to the KJO hospital, the patient will be
under the hospital management.
Once the patient is treated and released, he provides the concerned KJO department or
Contractor with a medical release form for fit for duty declaration and resume his
responsibilities on the job site.
After full treatment, copies of all hospital records made will be copied into the individual’s
record and kept in the case management file.
