Flexi 5 is a 5-pay peso-denominated variable life insurance product. Your living benefit is equal to the Account Value which is partially or fully withdrawable from
the policy's separate fund(s). The death benefit is either the Account Value or the policy's Face Amount, whichever is higher. The minimum death benefit is equal to
500% of the Basic Premium plus 125% of all the Top-Up Premiums less 125% of all withdrawals.
Your policy is a variable contract, therefore only the Face Amount of the policy for the corresponding Insurance Charge paid will be guaranteed. Other benefits,
namely the Account Value and the Death Benefit, shall depend on the investment experience of the separate fund(s) to which the policy is linked.

The Peso Fixed Income Fund seeks to provide regular interest income, consistent with its policy to preserve capital and to maintain liquidity of its
investments, through a diversified portfolio of high grade bonds and/or evidences of debt of the Philippine government or government-controlled
and owned corporation or solvent corporations and institutions.

The Peso Equity Fund seeks to maximize income consistent with its policy to preserve capital and to maintain liquidity of its investments through a
diversified portfolio of high-quality listed equity issues - blue chips and growth stocks listed in the Philippine Stock Exchange.

The Peso Bond Fund seeks to provide regular interest income, consistent with its policy to preserve capital and to maintain liquidity of its
investments, through a diversified portfolio such as Treasury Notes/Bills, Certificates of Indebtedness issued by the Bangko Sentral ng Pilipinas
and other government securities or bonds and evidences of indebtedness or obligations, the servicing and repayment of which are fully
guaranteed by the Republic of the Philippines or any of its instrumentalities.

NOTES:
1. All figures illustrated herein are in Philippine Pesos.
2. The basic premium charge is equal to 70% of the basic premium on the first policy year, 20% of the basic premium on the second policy
year, 10% of the basic premium on the third policy year and 0% thereafter. The top-up premium charge is equal to 4% of the top-up
3. Annual Insurance Charge starts at 1.65 per thousand of net amount risk based on the insured's attained age to be deducted monthly.
4. Annual Administrative Charge is Php 1,200 if within paying period and Php 600 after the premium paying period to be deducted monthly.
5. The illustrative values above relate to your basic plan only less any rider charges deducted from the fund value. It assumes that all premiums
are paid in full when due and as planned, and the current scale of charges remain unchanged.
6. The Fund Value will increase or decrease in accordance with the separate fund(s) experience.
7. Asset Management Charge of 1.75% for Peso Fixed Income Fund, 2.25% for Peso Equity Fund, 1.96% for Peso Bond Fund, 2.24%
for Cocolife Global Consumer Trends Fund, and 2.24% for Cocolife Asian Multi-Asset Income Fund, if chosen, is not included in the
above illustration.
8. For the Unitized Fund(s), a withdrawal charge equal to 30% of the amount withdrawn for the first policy year, 20% of the amount withdrawn
for the second policy year, 15% of the amount withdrawn for the third policy year, 10% of the amount withdrawn for the fourth policy year,
5% of the amount withdrawn for the fifth policy year and 1% of the withdrawn amount in excess of 1 free withdrawal for the sixth policy year
and every year thereafter shall be imposed.
9. We guarantee that your policy will continue to be inforce for the first two (2) years even if your Account Value may become insufficient to
meet the monthly charges due provided you were able to pay all premiums due and the sum of all withdrawals that you made is less than the
sum of all the Top-up Premiums. We will however deduct the unpaid amount from your account as soon as there is sufficient balance.
10. A loyalty bonus at the end of the 10th policy year and for every 5 years thereafter is included in the above illustration. The loyalty
bonus is equal to 1% of the average monthly Account Value balance of the preceding 5 years. This is non-guaranteed and may vary
depending on the actual performance of the fund.
11. The standard premium rate scale was used in the above computation. The application including additional net amount at risk due to top-up
premiums is subject to underwriting approval.
12. Values that appear in this proposal may differ from those of the policy contract due to small rounding off errors. The above has been
drafted by the soliciting agent for illustration purposes. While we have made every effort to ensure accuracy, errors and omissions
may have occurred in the preparation of this proposal. The terms and conditions that shall bind the parties are expressly provided for
in the policy contract.

DISCLOSURE OF CONFLICT OF INTEREST:


Our Investment Manager makes investment decisions for the Funds based on the circumstances of each Fund and independently of decisions made for
other funds. There are times when the Investment Manager may take the same investments for a Fund and one or more other funds. This may create a
conflict of interest if there is only a limited amount of the investment available, or if the investment is purchased at different times or at different prices
for different funds. If this happens, the Investment Manager will attempt to allocate the investment fairly between the Fund and other funds. Factors the
Investment Manager considers in allocations include the size and timing of previous allocations, whether the security meets the objectives of the
respective portfolios, the relative portfolio size and the rate of growth of the portfolios.

By affixing your signature below, you certify that you have read and understood the illustrations of this proposal. Moreover, you also certify
that the features, changes in the illustrative interest rate, the manner in which the variable benefits will reflect the investment experience of
the separate investment account(s) and the guarantees on the minimum death benefit were explained by your soliciting agent and were fully
understood. You also certify that you will assume all investment risks associated with the policy.

This illustration shall form part of the insurance contract once the policy is issued.
