AFFIDAVIT OF LOSS

I, RONALD M. DULDULAO, of legal age, Filipino, married, President of EMRD

Wells Cargo Movers Inc. and with postal address at Lot 2&5, Block 28 Ezra St. BF
Martinville, Manuyo Dos Las Piñas City., under oath, depose and state that:

1. That the registered vehicle of one (1) unit, KIA, is under of EMRD Wells Cargo
Movers Inc. described as follows:

2. As proof of ownership of the above-described Motor Vehicle, I was issued a Official

Receipt (OR) for the year 2022 by the Land Transportation Office of Pasay City,
Philippines.

3. That when I was about to register the said motor vehicle for year 2022, I could no
longer find said the original Official Receipt of the said motor vehicle.

4. That despite diligent search and efforts to locate the said document, I could no
longer find the same such that I believe that they are now lost beyond recovery.

5. As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing

and to support my application for the issuance of new Original Receipt, in lieu of the
ones that were lost and for whatever legal purpose it may serve.
