Directions: 1. Read the description of each Professional Behavior.

2. Review the behavioral criteria described in each of the levels (Beginning, Intermediate, Entry, Post-Entry) for each behavior.

3. Self assess your performance continually, relative to the Professional Behaviors, using the behavioral criteria.

4. At midterm and at the end of the clinical experience, complete this form.
a) Using a Highlighter Pen, highlight all criteria that describe behaviors you demonstrate in Beginning (column 1), Intermediate
(column 2), Entry Level (column 3) or Post-Entry Level (column 4) Professional Behaviors. Use different color pens to denote
midterm vs. final.
b) For each behavior, identify the level within which you predominately function at midterm and again at final.
c) Document specific examples of when you demonstrated behaviors from the highest level highlighted.
d) For each Professional Behavior, list the areas in which you wish to improve.

5. Share your self-assessment with your clinical instructor at midterm and final, specifically seeking his/her feedback on your

6. Have your CI sign that they have read and discussed your self-assessment (last page); sign and return to the DCE/RCC at the

**Professional Behaviors were developed by Warren May, Laurie Kontney and Annette Iglarsh (2010) as an update to the Generic Abilities.

Examples of behaviors to support my self-assessment: I feel that I am able to come up with relevant and appropriate questions regarding a specific case. I make
sure to look over all of the available information I have at my disposal, so I know the general and important information regarding a patient. I do recognize that
there are some things that I am not confident in so that is when I ask for help or advice about an idea I may or may not have. However, I try to come up with all of
the possible ideas I can regarding a patient case so I can be prepared for any case scenario.

Regarding this Professional Behavior, I would like to improve in the following ways: I would like to be more aware of the scientific literature and research that
supports treatment or examination questions/exercises. This would benefit me because I could apply it to a patient case if I had read up on a similar case report or
evidence-based research paper.

Examples of behaviors to support my self assessment: I have gained the confidence to be able to analyze data gathered from the patient history and examination

and apply it to their treatments. I combine what I have learned with my didactic knowledge, evidence-based research, and clinical experiences to create new ideas
based on the patient case. I am open to new ideas while working with the patient and modify the treatment as needed.

Regarding this Professional Behavior, I would like to improve in the following ways: I would like to be able to come up with alternative ways the treatment could go,
or patient could present. I have realized that the patient could present in a completely different way each session and would need to be prepared for their
treatment.

Examples of behaviors to support my self-assessment: It is important to have legible handwriting and full sentences with accurate spelling especially when
other audiences will need to read and understand my work. I understand that non-verbal communication is just as important as verbal communication
because facial expressions can sometimes say more than words. Showing that I am confident will allow a trusting relationship to be formed between me and
my patient and colleagues. I also understand that having professionalism with electronic communication is important especially when speaking to other
healthcare professionals. Working collaboratively with others is important because we can share ideas and talk through those that we are uncertain with.

Regarding this Professional Behavior, I would like to improve in the following ways: I would like to be able to collect all of the necessary information from
patients in a smooth process, whereas right now I feel that I struggle with the flow of my subjective portion. I also need to work on providing effective
education while asking questions so I can be more efficient in my time with the patient.

Examples of behaviors to support my self assessment: I feel that I have gained a better understanding of what information is necessary to gather while taking
a subjective evaluation. Since there is only a certain amount of time I have with a patient, I need to make sure to focus on the pertinent information for their
case. I have also gotten more efficient in terms of writing down the important information they relay to me during the evaluation. When they seem to get off
track, I do my best to get the conversation back on the topic of why they are here.

Regarding this Professional Behavior, I would like to improve in the following ways: While I am able to steer the patient in the right direction during
conversations in an evaluation, I would like to improve on changing topics and keeping the important information being talked about first. I find it difficult
sometimes when the patient wants to talk about things other than their diagnosis and have to reel them back into the conversation so I am able to receive all
of the information I need.

Examples of behaviors to support my self-assessment: When a problem arises, it is important to state it clearly when talking about it with a patient or
colleague, so you are able to work together on coming up with solutions to that problem. I use outside resources to the best of my ability when I am stuck on a

problem and find the best way to solve it. I make sure to prioritize problems and talk with others to figure out what could be the cause and how to address the
situation effectively and efficiently.

Regarding this Professional Behavior, I would like to improve in the following ways: I would like to improve on being able to expand my knowledge to come up
with many possible solutions or causes of the problem so I can have different directions to go. I would also like to be able to utilize resources (research
articles, case studies) and come up with consequences that could occur so I can be ready for whatever could happen.

Examples of behaviors to support my self assessment: While the patient is talking about their diagnosis, I mentally come up with ideas or ways I could treat
them and what tests or measures to perform. I try to identify other factors that could be taking place and discuss them with the patient to see if they are
related. While the patient is relaying this information, I try my best to come up with possible solutions and ways we can go about their treatment, so they are
aware of what to expect during their sessions.

Regarding this Professional Behavior, I would like to improve in the following ways: I would like to improve on being more comfortable with mentally thinking
about their diagnosis and information they are telling me so I can easily relay information to the patient. There have been times when the patient is unsure of
why they are in therapy and what the outcomes could be so I want to be able to tell them the benefits and what outcomes they will see and expect to go
through during their therapy sessions.

4. Interpersonal Skills – The ability to interact effectively with patients, families, colleagues, other health care professionals, and the community in a

Examples of behaviors to support my self-assessment: I feel that my interpersonal skills are what I am most comfortable with because I love having good
interactions with patients during their session. I demonstrate interest and respect to patients during conversations and try my best to establish trust within
the first interaction.

Regarding this Professional Behavior, I would like to improve in the following ways: I would like to become more familiar with others’ emotions and non-
verbal communications when they are talking with me. Sometimes I find it difficult to know when a patient is joking around or being serious about certain
topics, but I want to be able to have a better understanding of the point they are trying to get at. Since everyone has a different definition of verbal and
non-verbal expressions, I want to improve on knowing what to look for when interacting with different patients.

Examples of behaviors to support my self assessment: I feel that I have gotten more confident and comfortable in terms of reading a patient’s body
language and non-verbal communication. This has been beneficial especially during an exercise because I am able to tell if the patient is pushing
themselves too hard or if they are able to continue. I have been able to build connections with the patients, which has allowed the treatment sessions to
run smoothly and allowed for me to accurately tell how the patient is responding to treatment.

Regarding this Professional Behavior, I would like to improve in the following ways: While I have not experienced many challenging or unexpected
situations or interactions, I would like to be able to know how to be prepared to handle this type of situation when I come across it. I want to know how to
handle a patient during a challenging time and what is expected of me so I respond carefully and professionally.

Examples of behaviors to support my self-assessment: I am always sure to be on time, take responsibility for my actions, and show my willingness to
learn and be open to new ideas. Taking responsibility is not always easy, but I have always known that taking responsibility is one of the most important
tasks, especially in the career field. I also make sure to be aware of sensitive topics or populations when working with patients, to which I collaborate with
other team members or families in some instances.

Regarding this Professional Behavior, I would like to improve in the following ways: Since I have not been given projects, I cannot say one way or
another that I would not need prompting. However, if I were to be given a project, I would try to get it done/work on it without needing to be prompted. I
also find it difficult to delegate tasks since I am still learning myself, so I would like to improve on that in the future as well.

Examples of behaviors to support my self assessment: I feel that I have gotten better in terms of preparing for patient cases and documenting in a timely
manner without being prompted to do so. I have also found ways to ensure patient accountability to their treatments since there are people who do not
always want to come. I encourage them to continue with their sessions and home programs, so they continue to see progress.

Regarding this Professional Behavior, I would like to improve in the following ways: I would like to be able to know when to refer a patient to other health
care providers and how to do so. While it is not in my power yet, I would like to be aware of what to look out for and how to properly talk to the patient
and their healthcare provider in the best way possible.

Examples of behaviors to support my self-assessment: Professionalism is something that is extremely important, especially in the health field and I have
been sure to be as professional as possible since starting this program. There have been a lot of new things that I have been exposed to since starting
this clinical rotation, and I have made sure to maintain my professional image to ensure that I am putting in all of my effort and commitment to this field. I
also am aware of others around me and take note on their professionalism and apply it to my own.

Regarding this Professional Behavior, I would like to improve in the following ways: I would like to improve on knowing when professional decision
making would require the help of others since I have not been involved in many professional decisions. I would also like to learn more about the societal
expectations of this profession to ensure that I am aware of them and could discuss in the future with co-workers or classmates.

Examples of behaviors to support my self assessment: I feel that I have gotten more comfortable collaborating with other people of the team to ensure
the patient is receiving the best possible care. Since the patient is seeing other therapists, I have learned that talking with them about their progress is
equally as important as seeing it yourself. By collaborating with others, it allows new ideas to form, and we can continue to progress the patient.

Regarding this Professional Behavior, I would like to improve in the following ways: I would like to improve on expanding my scope of practice and
collaborating with other healthcare professionals and those in professional practice to ensure the patient is being provided with the best and proper care
to their diagnosis.

Examples of behaviors to support my self-assessment: I feel that constructive feedback is important because I have found that I learn best from my
mistakes. When I receive constructive feedback, I am sure to take it into consideration and try my best to improve on it the next time, to which I ask for
feedback again. I always make sure to demonstrate receptive behavior to ensure that I am listening and will be developing a new way to go about the
task I previously completed. I also make sure to critique my own performance to know what to improve on for the next time.

Regarding this Professional Behavior, I would like to improve in the following ways: I would like to be able to come up with goals on my own with minimal
critiques. I would also like to improve on the amount of time it takes for me to fix something that was given constructive feedback on to help with my
efficiency.

documentation, and overall care to the patient. I have been able to seek feedback from others and modifying the ways I go about treatments or
evaluations to ensure I am being as effective and efficient as possible, while providing the best care.

Examples of behaviors to support my self-assessment: I always make sure to come as prepared as possible and ask for help when needed. I make sure
to talk with others when I am stuck on a problem and share ideas that I have come up with to ensure I am providing the best possible care. I try to use as
much evidence and effective methods as I can, as well as trying to progress previously used ideas to challenge the patient in their treatments.

Regarding this Professional Behavior, I would like to improve in the following ways: I would like to improve on initiating learning opportunities outside of
my scheduled time as well as come up with strategies I can discuss with my colleagues. I would also like to improve on talking with other disciplines or
how to properly seek referrals without being hesitant.

Examples of behaviors to support my self assessment: I feel that I was able to take time out of my scheduled clinical time to seek ideas on how to treat
more complex patient cases and read more about the McKenzie method. I was provided with books regarding the McKenzie method and was able to
learn more about it as I went along in this rotation. I feel that I have also realized when a good time would be to seek out other disciplines for a patient
(ex: SLP or OT).

Regarding this Professional Behavior, I would like to improve in the following ways: While I feel like I am gaining more confidence in terms of knowing
when a patient would benefit from other disciplines, I would like to improve in terms of talking with the patient and other healthcare professionals as to
why and what they could gain from it.

Examples of behaviors to support my self-assessment: I have learned it is important to recognize my own stressors because I need to be able to handle
them in the best/healthiest way possible. I make sure to seek assistance but also maintain a professional demeanor at all times. Maintaining a balance
between my professional and personal life was something that I struggled with previously, but I feel that I am getting a better handle on it, as well as
establishing ways to cope with my stress.

Regarding this Professional Behavior, I would like to improve in the following ways: I would like to improve on knowing how to actively employ stress
management techniques in any environment or situation. I would also like to be able to differentiate and understand inconsistencies in the educational
process and know how to handle them in a given time.

Examples of behaviors to support my self assessment: I feel that I was able to manage my stress throughout this clinical in a better way and make it
more of a learning experience. While there are many times it would be easy for me to feel overwhelmed, I was able to take a step back and understand
what is most important and take it one step at a time.

Regarding this Professional Behavior, I would like to improve in the following ways: While I have not undergone an emergent situation, I would like to
become familiar in terms of how to properly handle a situation like that. I feel that it would be very easy to become overwhelmed in an emergency
situation, but I would like to be able to be as calm as can be and know exactly the right steps to take in terms of how to handle it.

Examples of behaviors to support my self-assessment: I feel that I am able to prioritize more important information and develop smaller components for
large questions, so it is easier to comprehend and take into consideration in pieces rather than as a whole. I also am always willing to take on new
learning opportunities and compare my own learning based on the previous experiences that I have been faced with. I try to always apply new
information whenever possible and know that there is most likely never one answer to a problem, to which I need to be open to finding as many solutions
as possible.

Regarding this Professional Behavior, I would like to improve in the following ways: I would like to improve on using literature and research articles to
support my ideas and come up with new solutions to problems. I would also like to be more involved with research and case studies so I can become
more interested and utilize those resources more often.

Examples of behaviors to support my self assessment: I feel that I have been able to seek outside literature in terms of coming up with new ways to go
about a treatment session. There have been some patient cases that I do not feel as comfortable in, which I turn to outside literature and the skills I have
learned throughout my time in this rotation.

Regarding this Professional Behavior, I would like to improve in the following ways: While I have utilized some outside sources, I would like to continue to
expand my thoughts and read up on more articles that are similar to patient cases.

Based on my MIDTERM self-assessment of my Professional Behaviors and the areas I have identified for improvement, I am setting the
following goals (list at least 2):

1. I would like to utilize outside resources such as research articles or case studies more often to come up with more ideas and solutions to
problems that may arise within a specific case.
2. I will become more aware of verbal and non-verbal emotions of patients and know when it is best to seek for outside help or if I am able to help
them on my own.
To accomplish these goals, I will take the following specific actions:
I will utilize outside resources when given a patient case to try and compare other methods or ways of treatment that was already used and compare
it to the outcome of my patient. I will read up on case studies and research articles to provide myself with many solutions and possible causes to become
more aware of what a patent might be dealing with and how I can help them.
I will be more aware of how patients are responding to treatment or certain questions I ask in terms of how they are feeling or more personal
questions. I will take into consideration their body language and facial expressions to ensure that I am fully aware and if I notice anything that is unusual.

Based on my FINAL self-assessment of my Professional Behaviors and the areas I have identified for improvement, I am setting the following
goals (list at least 2):
1. I would like to be more comfortable in terms of guiding the subjective evaluation in the direction of important and pertinent information only. I
have found it is easy for a patient to get off topic, but I want to be able to be comfortable steering the patient back in the right direction.
2. I would like to be comfortable if I come across an emergent or challenging situation with the patient.

To accomplish these goals, I will take the following specific actions:


I will become more confident in how I go about a subjective evaluation and make sure to only ask specific questions related to the case. I will allow
the patient to go off topic when applicable but will be comfortable in terms of guiding them back into the important questioning and conversation.
I will become more aware of different types of emergencies that could come about in the clinic. I will be sure to keep my BLS renewed and attend
other classes if able to ensure I am as prepared as can be in an emergency situation.
