PARENTS’ CONSENT FORM

I allow my daughter/son _ __________________ from __________________taking

up __________________ of BOHOL ISLAND STATE UNIVERSITY MAIN

CAMPUS to attend the FIRST ASSEMBLY & TEAM BUILDING of the

ARCHITECTURAL DRAFTING ASSOCIATION (ADA) this June 16, 2023, which

will take occurrence at Bikini Beach, Biking, Dauis, Bohol, from 7 in the morning

to 5 in the afternoon.

Given the aforementioned circumstance, we are giving our full consent to our

daughter/son to attend the said activity. I understand and agree that the organizers

will secure the safety of the participants and the smooth conduct of the activity.

However, in any case of unprecedented happening, I will not hold the school

responsible.
