Reference:- 1, Memo No HF/SPSRC/19/2020/79 dated 13/05/2020 (Guidelines for smooth running of Dialysis Services in the state) 2, Memo No HF/SPSRC/19/2020/84 dated 29/05/2020 (Guidelines for Pvt Hospitals regarding Dialysis Services inthe state ) 5. Memo No HPH/9M-21/2020/110 Dated May 22, 2020 ( Revised discharge policy for COVID-19 ) 4. Memo No HFW-14015(17)/12/2020-SPSRC SEC -Dept of H&eFW/PPP/1305 dated 28-07-2020 (Equipment and Logistic management for COVID/pre COVID dialysis units of State Department of Health & Family Welfare has a established chain of Dialysis services both run departmentally as well as in Public Private Partnership model covering every corners of the state. Also as part of COVID-19 management, several new Dialysis units have also been set up in COVID Hospitals to deal with COVID positive as well as suspect COVID dialysis cases. MT Dialysis has been deployed. Relevant guidelines and orders issued from time to time. (Reference above). Now considering the recent surge of COVID cases including increased requirement of COVID Dialysis cases, the following.instructions may be followed stricty:~ 1, Admission of COVID positive patients with CKD (Chronic Kidney Diseases) and requiring Dialysis may be done at COVID Hospitals where dedicated COVID Dialysis units are functional. Accordingly the admission cell of state / district may advice for admission, (Updated Lists of all Dialysis units including dedicated COVID Dialysis units are attached in Annexure-A). Example:- A CKD patients of North 24 Parganas, if found COVID positive and requiring admission, may be referred to GNRC or COM Sagar Dutta both of which have functional COVID Dialysis units COVID positive patients with CKD (Chronic Kidney Diseases) who are in home isolation may require Dialysis. These patients may receive Dialysis on day care basis in any dedicated COVID dialysis unit with pre infornfation to hospital. 3. Nephrologists have been nominated for each district who will act as mentors and provide Technical guidance to the District as and when necessary. The revised list with Contact no is shared in Annexure- B 4. IF COVID Dialysis has to be conducted in any PPP Dialysis unit, then necessary guidelines as per Memo No HF/SPSRC/19/2020/79 dated 13/05/2020 (Guidelines for smooth running of Dialysis Services in the state), has to be followed. Hospital authorities are requested to instruct district BMEs for regular check up of machine maintenance issues. MT’ Dialysis posted will take care of all other aspects including placing indent for logistic! consumable through concerned Hospitals. Requisitioned private hospitals are functional and more new units are coming up. In government setups equipments, HR and logistics are provided by department. There are certain requisitioned private hospitals also where department has provided similar support. Technical support and certain logistics are being supplied by department through WBMSCL, as per sd by local units. Most of remaining CAT items are being arranged by Hospital authorities/ District at indent their level, Now we may request MSVPs and CMOHs as well as Nodal officers of all such COVID positive/’suspect Covid Dialysis units to follow the guidelines provided:- 1. For government run units the central supply of certain proprietary logistics and equipment by WBMSCL may continue while CAT items to be procured by hospitals as per norms. Hospital / District authorities may also procure the proprietary Non-CAT items from available funds following financial norms. 2. For requisitioned private hospitals where the dialysis is running with departmental HR and logistic the same may continue till the above are taken back when Covid situation subsides. However there will be no payment in this case for dialysis service. The bills may be scrutinized to rule out any claim towards Dialysis services in these hospitals. 3. All units will send timely indent to WBMSCL with a copy to PPP CELL well with advance with proper UC. In this regards a brief SOP is attached in Annexure-A. Same may be followed. List of functional COVID Dialysis/ suspect COVID units are attached in Annexure B for reference. This will bbe updated from time to time, 4. All other guidelines and advisory issued from time to time regarding Dialysis services has to be followed strictly. Strict vigilance regarding Infection Control Protocol and BMW management to be maintained. This is issued in interest of public services. For any issues related to equipments (HD machines SLED/ routine machines and portable RO plant / fixed RO plant) breakdown/ maintenance in the dedicated Dialysis units, the MT Dialysis should immediately contact the OEM (Original equipment manufacturer) and send a mail to pppdialysisl@gmail.com and procurementewbmsc.gov.in. b, Regular up keeping of machines and RO plant is responsibility of the posted MT Dialysis even if there are no cases. Any issues or problem detected should be brought into notice of the Nodal officer of the unit as well as concerned OEM and BME of the district. BME will be responsible for co-coordinating matter with OEM along with MT Dialysis and also WBMSCL experts. ©. For all those COVID Dialysis run by private partners as per Government orders ( Kolkata MCH/ MR Bangur DH) , they will be solely responsible for all HR/ Logistics and will raise bills for service as per approved rates. d. Indent along with UC to be compiled and forwarded by Hospital in-charge through nodal person of particular district / MSVP to the dept with a copy to WBMCL / PPP cell. Allotment will be for 1 month period based on UC and stock availability at HQ. Also based on the above status procurement planning, will be done for further stock by WBMSCL. Further indent to be done by local units when 70% stock consumed. Please note that UC should reflect stock position of all items at DRS if any as well as Local Dialysis units. Also as part of COVID-19 management, several new Dialysis units have also been set up in the Level-2 and Level-4 COVID Hospitals to deal with COVID positive as well as suspect COVID dialysis cases. MT Dialysis has been deployed and logistics provided. Orientation Training of Technologist also done by State level experts. 2. There are reports that Dialysis services are being disrupted due to closure of units on ground of Dialysis, patients being tested positive for SARS-Cov-2 and staffs going in quarantine for which a uniform guidelines is mandated, There are also request from districts unit to provide Nephrologists guidance and support. 3. To maintain uninterrupted dialysis services, the guidelines attached in Annexure-A is to be followed. 4, Accordingly support will be provided by nominated Nephrologists for each district who will act as mentors and provide Technical guidance to the District as and when necessary. The list with Contact no is shared in Annexure-B. 5. However for PPP units, the Nephrologists attached to these units as per agreement will monitor their units. A compliance report has to be send to the Hospital authorities with copy to the Department every 7 days, duly certified by attending Nephrologist. Hospital level Dialysis monitoring team will conduct overall monitoring and departmental experts will provide hand holding support for all Dialysis units including the COVID Hospitals. 6. Testing of medical staff who conducted Dialysis of the Positive COVID case detected later, may be done as per Advisory for managing Health care workers working in COVID and Non-Covid areas of hospital The case has to be categorized as High Risk exposure ( HCW without face mask/face-shield/goggles having face to face contact with the case within I meter for more than 15 minutes or having exposure to body fluids) or Low risk exposure ( Contacts not meeting above criteria) For High Risk exposure Staff to be quarantined for 14 days and tested as per ICMR testing protocol and managed as per standard guidelines. Low risk exposure will continue to do the work and self monitor their health for development of symptoms. This is applicable to Dialysis Technologist attending the patients. However High risk exposure is to be avoided at all cost. Until test results received of Technologist with High Risk Exposure, those who are Low risk exposure/ contacts and non contacts will resume / continue Dialysis services after proper sanitization of the unit. Regarding testing, of other Dialysis patients attached to the same unit, the patients receiving Dialysis in adjoining beds within a distance of 3 ft in all directions in that particular shift (Last contact / Last shift when the positive case received Dialysis) may be tested for COVID-19. Routine testing off all patients in that shift is not recommended. Patient wise Bed allocation matrix in each shift to be maintained and record saved. For all the Dialysis cases tested for COVID-19, as contacts of known case as above, the dialysis may be conducted in any nearby unit with Isolation Dialysis facilities as per Annexure-B. For those without any nearby dedicated isolation unit, Dialysis of COVID suspect/ SARI cases may be done in last shift in empty unit with full PPE suit and unit disinfected as per protocol before resumption, Guidelines to be followed day to day strictly in all dialysis units:- Proper fever screening, for all dialysis cases to be ensured. All patients to enter unit after hand sanitization. A sign board should be posted prominently in Bengali, English and local language asking patient to report any fever, coughing or breathing problem in the dialysis unit and waiting area. Patient should mandatorily wear mask (triple layer surgical mask throughout Dialysis duration) and do proper hand washing before entering unit. Any meals inside unit are to be strictly avoided. 3|Page 5: 10. n 12. 13. 4. 15. . Designated screening area with adequate space for social distancing to be maintained with display of IEC material (Posters etc.) for COVID-19. Dialysis Technologist, staffs and caregivers should work with personal protective equipments as per standard guidelines. It is recommended that even for routine Dialysis they should wear Triple layer surgical mask, Face shields, Double gloves, Gown/Macintosh, head cover. A Dedicated Isolation area with proper partitioning should be used for conducting COVID suspect Dialysis or COVID Positive cases. The patients belonging to this category are to be dialyzed in nearest designated unit ( Annexure-B) For those without any adjoining dedicated isolation unit, Dialysis of COVID suspect cases may be done in a separate shift preferably the last shift and unit disinfected as per protocol before resumption. For cases referred from any other Government and private hospitals to the designated Level-2 COVID hospital for Dialysis, asymptomatic COVID suspect cases are to be dialyzed as day care procedure. SARI/ symptomatic COVID suspect may be admitted as per diagnostic criterias. Staffs conducting Dialysis for Suspect COVID/COVID positive cases should use complete PPE suit and should not look after any other patient during the same shift. It should preferably be conducted in last shift. Staffs should also be trained in proper use of PPE. A separate area for donning and doffing needs to be demarcated. Stethoscope Diaphragms and tubing’s should be cleaned with alcohol based disinfectant including hand rubs in between patients. Disinfection and disposal of bio medical waste to be done strictly following existing protocol. After each shift all exposed surface like cot rail, fluid stands, machine surface should be wet mopped with 1% hypochlorite solution or alcohol based solutions. All Universal precautions to be followed. In this regards SOP for Dialysis already circulated may be followed as reference. Regular Health checkup of staffs to be done by Hospital Authority. A grievance redressal mechanism has to be implemented and all patients guided accordingly. Regular sanitization of the entire unit to be done. Hospital level Dialysis monitoring team will conduct overall monitoring and departmental experts will provide hand holding support for all Dialysis units including the COVID Hospitals as and when necessary. For PPP units, the Nephrologists attached to these units as per agreement will monitor their units. A compliance report has to be send to the Hospital authorities with copy to the Department every 7 days, duly certified by attending Nephrologist. 4|Page III. Specific Guidelines for Private Dialysis units :- (Over and above the guidelines in I & II) @ 1. dedicated facilities or zone for Dialysis of COVID suspect cases/ Asymptomatic cases awaiting testing is not available with private hospital, patients may be referred to nearest unit where such services available. The designated Government facilities are attached in Annexure-B for reference. 2. A Nodal Officer (Infection Control Officer) shall be identified by the hospital, responsible for implementation of Infection prevention and Control (IPC) activities who will also address all matter related to Healthcare Associated Infections (HAIs). 3. Nephrologists attached to the private organization will be responsible for strict protocol implementation and proper orientation training of all the staffs associated with dialysis activities in PPP units from time to time. 4. A compliance report has to send to the District CE Authority with copy to the Department every 7 days, duly certified by attending Nephrologists and counter signed by Medical Superintendent. 5. Safety and protection of Medical Technologist and associated staff in dialysis units of private hospitals need to be given the highest priority. This includes PPE, administration of HCQS as prophylaxis as per ICMRs Guidelines.
