1. We personally know parents, (Justine Scott, for brevity) because we are friends and
neighbors for a long time.

2. Justine Scott was born on April 20, 2020 at Community Quarantine and later on
General Community Quarantine due to the pandemic COVID19 at the time of his
birth. There is a difficulty in movement and this affected the registration process.

3. We are not in any way related to affinity nor consanguinity;

4. That finally, we are executing this affidavit for the purpose of informing the
authorities especially the Local Civil Registrar and the Philippine Statistics Authority
of the above-mentioned and to support the application for late registration of birth
certificate or
