Suggested Format in Submitting PROJECT PROPOSAL for Income Generating

 EXECUTIVE SUMMARY:

Provide a brief overview and summary of the project. It should provide a credible statement that
describes your project and the significance of establishing it.

PROBLEM STATEMENT:

State the challenges identified, the rationale/need to address these challenges and the conditions
to be changed by the project  

PROJECT OBJECTIVES:

The objectives should describe the intended outcome of the project and should be SMART
(Scientific, Marketable, Adaptable/Applicable, Realistic and Transparent)

Indicate how the objectives will contribute to the achievement of the project, what difference the
project will make and the time frame during which this will happen.  

ENVIRONMENT ASSESSMENT:

This part describes the biophysical environment and possible impact of the project on it.
Mitigating measures to reduce the negative effects have to be identified and included in the
economic analysis.

PROJECT IMPLEMENTATION AND MANAGEMENT PLAN:

Describe the project activities indicating how the objectives will be accomplished, what will be
done, who will do it, who are the implementers, partners and beneficiaries and when  it will be
done.  Describe how the project will be sustainable after the funding period.   

(Please state the time schedule of the laboratory for the project and the list of pupils/students
involved in the project. Documents the orientation/meeting with the parents of involve
pupils/students)

PROJECT MONITORING AND EVALUATION:


Describe how you are going to monitor and evaluate the project so as to assess progress during
implementation and improve the project efficiency as the project moves along. 

DOCUMENTATION AND SHARING RESULTS :

Describe how you are going to document the progress of your project during and after
implementation.  State how you will document and share your results and let others know of
your purpose, methods and achievements. 

(State the percentage of profit sharing for pupils/students and school.)

PROJECT BUDGET:

Indicate the total cost of the project and also provide a detailed budget for these costs.

Provide any additional information in support of this proposal application 

(Pictures of Garden Site, Meeting with the Learners Involved in the Project, Parents )

Reminders: Use A4 bond paper, observe proper margin Left -1.5 inch, Top 1 inch, bottom-1
inch and Right -1 inch, spaces (double spaces for every paragraph and single spaces for
sentences), highlight the title and important words , use font type - times new roman or Tahoma
and font size 12.

Note:

1. After completing your proposal in the above format  submit it to the Division Office
for SDS approval with your letter of request (see sample above) to conduct an
income generating project.
2. It is expected that the proponent of the IGP’s will sustain the projects after using it
for professional growth , for the benefits of the pupils/students as well as the school.
3. Expect the monitoring of the IGP’s from the Division Office.
4. Always be ready with the documents (written, pictures or videos) during the
monitoring and evaluation anytime after establishing the projects.
