3. Authenticated copy of certificate of Civil Service eligibility/ PRC license;
4. Photocopy of Transcript of Records and College Diploma;
5. Photocopy of Training Certificates preferably with training course outline; and
6. Photocopy of Employment Certificates with detailed job description.
